Pharmacy Community Health Worker (W. Providence)

Asthenis Pharmacy is located in the West End portion of Providence. Serving clients of all backgrounds, we are a community resource for health education outside of your doctor’s office. The services we provide include: prescription pickups, review of your medications with a pharmacist, general health education, and delivery of medications to your home if needed.

ESSENTIAL JOB FUNCTIONS:

●Establish relationships with trusted community leaders to identify and document cultural and structural barriers that interfere with clients accessing community resources

●Assess the barriers that exist for individuals who are in need of healthcare/social services and provide language, transportation, and/or employment support as needed

●Relay provided health care education/information to community residents addressing frequently asked questions related to hypertension, diabetes, and managing complex diseases

●Gain insights about needed resources and changes required to improve assistance for community residents

QUALIFICATIONS:

‘●Community Health Worker in Rhode Island, or actively working to become certified preferred

●High level of interpersonal skills and excellent writing and oral communication skills

●Fluency (speaking, reading, and writing) in English and at least one other language

●Strong interpersonal skills

●Comfort working with diverse communities
