Community Health Worker

Part-time: 5 Days a Week x 25 Hours

Job Summary:

The Community Health Worker plays an integral role in helping elders, women, youth, and families apply
for basic needs, including housing, food, medical, and social services. The Community Health Worker
will assess the participants’ needs, provide necessary interventions, and evaluate whether the participants’
needs have been met. The Community Health Worker is encouraged to collaborate with other community
agencies for support. Additionally, our Community Health Worker will assist with HGI’s Crush COVID
program, Rhode Island Life Index program, along with other activities and programs as needed.

Duties and Responsibilities:

Coordinate and provide culturally relevant care that is safe, timely, effective, efficient, equitable,
and participant-centered

Handle case assignments, draft service plans, review case progress and determine case closure

Help elders & families achieve wellness and autonomy

Facilitate multiple care aspects (case coordination, information sharing, etc.)

Help elders and families make informed decisions by acting as their advocate regarding their
medical status and treatment options

Develop effective working relations and cooperate with team throughout the entire case
management process

Arrange relationships with social services, health and governmental agencies

Be proactive in your interactions with the elders and families as necessary to keep track of their
progress and to ensure satisfaction

Record cases information, complete accurately all necessary forms and produce statistical reports

Promote quality and cost-effective interventions and outcomes

Assess and address motivational and psychosocial issues

Demonstrates and integrates a strong understanding and commitment to the Mission and Values
Statements of HGI

Develop relationships with the Sweetie Care elders and families by spending time with them
during program services and activities

Educated community members, elders and families about crushing COVID, vaccines and etc.

Conduct in-person (socially distant) and/ or virtual RI Index surveys via phone and/or video
conference, and home visits with those with lack of technology

Other duties as instructed to serve our program participants

Minimum Requirements:

Must be reliable, honest, and self-motivated, and able to motivate others

Must be patient and respectful

Must be willing to learn and adhere to participants cultural norms

Sensitivity, discretions, and the appropriate handling of confidential information

Must be a team player and willing to take and follow supervision

Positive, open, caring, non-judgmental and no patronizing demeanor

Excellent judgment and decision-making abilities

Ability to handle and address participants issues and resolve participant needs

Strong organizational, time management, written and verbal communication skills

Computer literacy in MS Word, Excel and Outlook/Gmail

Current license, certification or registration

BS degree or Diploma in related field

Bilingual, a plus
