  • Participate in the SCHH COVID Response and Resilience Implementation Team and work in collaboration with partnering organizations.

  • Collaborate with SCHH nurses, PT, OT and team leads, PC nurse care managers, other care team members (Medical Assistants, Nurses, etc.), hospitals, partner agencies, members of the CHT in the patient’s network of care to improve patient care. 
  • Accompany patients to doctor’s appointments, assist with food planning/shopping, picking up prescriptions at the pharmacy, completing forms for benefit applications, and assist with other tasks as needed that support their medical, behavioral health and social needs.
  • Engage patients and families in health coaching efforts to develop and maintain health & wellness.
  • Attend CHW CORE training and/or Specialty training as required and appropriate to the population served, e.g. Older Adults, CVD/DM, Chronic Pain Self-Management.
  • Achieve and maintain certification
  • Attend monthly networking meetings convened by CHWARI for support, collective problem solving and share resources.
  • Participate in the implementation of high impact strategies and activities to demonstrate the effectiveness of CHWs.
  • Consider opportunities to participate in SCH leadership meetings (such as huddles) as CHW representative to lend professional experience.
  • Participate as CHW representative in COVID response and recovery planning tables and other RIDOH sponsored meetings to regularly share community trends and support initiatives of the project.
  • Participate with RIDOH in deployment to places where vaccine outreach is happening
  • Support SCHH in implementing COVID resilience and response protocols and policies/procedures.
  • Participate in qualitative and quantitative data collection to evaluate effectiveness of CHW strategies on client needs, services provided and other data points of interest.

QUALIFICATIONS FOR POSITION: Certification in Community Health Worker or working toward certification within 12 months required.  Lived experience related to the population served by South County Home Health.

GENERAL SUMMARY:

The Sexual Assault Victim Advocate provides a vital and urgently needed
service for victims of sexual assault and domestic violence in Rhode Island. With the Assistance
of an advocate, victims feel supported and empowered to continue with the rest of the criminal
justice process. Additionally, victims have access to services that are specific to their needs and
can assist them with the healing process. Victims who feel supported and are receiving services
geared towards holding more offenders accountable for their actions.


PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Responsibilities will include working directly with victims of domestic violence, sexual
    assault and stalking, providing follow-up advocacy, safety planning, support, shelter
    referrals.
  • Provides in-person support to victims at local hospitals with or without police.
  • Provides in-person support to victims at police departments
  • Provides in-person support to victims at court for hearings and restraining order issues.
  • Provide training, outreach and relationship building with various community
    organizations and agencies in our community.
  • Maintain data and providing program reports.
  • Should conduct a minimum of two outreach and training activities a month whenever
    possible.
  • Provides a positive, professional and enthusiastic image of the agency to the community

Job Summary:
Higher Ground International (HGI) has been awarded funds from the Rhode Island Department of Health to serve as a community partner in the design, implementation, and evaluation of strategies and activities aimed to train, deploy, and engage Community health Workers (CHWs) towards COVID-19 response and recovery efforts and towards building and strengthening community resilience through addressing existing health disparities.


The Community Health Worker will work in the community and with our program participants to assess program participants’ needs, provide necessary interventions, and evaluate whether the participants’ needs have been met. The Community Health Worker will provide community outreach and collaborate with other community agencies for support. Additionally, our Community Health Worker will assist with HGI’s Health and COVID related programs, including case management services, coordination of healthcare and social services, culturally appropriate foods distribution, along with other activities and programs to help improve the quality of life for members of the community.


Duties and Responsibilities:
❖ During the first year, the CHW must be trained and fully certified in the Community Health Worker Association of Rhode Island (CHWARI). This training and certification will be provided by the RI DOH.
❖ Attend the new public health training for CHW.
❖ Attend at least one of four specialty trainings and certifications for cross training of CHW.
❖ Educate community members, elders, and families about COVID related programs, vaccines, and other health/social services.
❖ Support elders, community members & families as they work towards achieving personal wellness and autonomy
❖ Develop individualized case management and care coordination plans for program participants and facilitate access to various supportive services and programs
❖ Coordinate and provide culturally relevant care that is safe, timely, effective, efficient, equitable, and participant-centered

A Health Equity Zone (HEZ) is an innovative, place-based approach that brings people together to build healthy, resilient communities across Rhode Island. The 02907 community has seen a disproportionately high number of COVID-19 cases as compared to other parts of Providence and across the state of Rhode Island.

POSITION SUMMARY: The Community Health Worker (CHW) is a vital member who will apply their deep understanding of the experience, language and/or culture of the community they serve to carry out one or more of the following roles: Provide culturally appropriate health education, information, and outreach in community-based settings, such as homes, schools, local businesses, and community centers. Serve as a bridge between individuals, communities and health and human services, including actively building individual and community capacity in 02907. Advocate and assist with 02907 resident and community needs. You will work with the HEZ and Rhode 2 Equity team implement initiatives that increase agency visibility within the community including distributing goods and services for COVID-19 mitigation, resilience and recovery. REPORTING STRUCTURE: The CHW will report to the HEZ Program Manager.

ESSENTIAL JOB FUNCTIONS:

  • Complete administrative tasks on an ongoing basis to ensure proper and organized record keeping, working with staff member’s when/if necessary.

This is a 5-year award comparing treatments for overactive bladder in women, with an innovative component dedicated to strengthening community outreach, engagement, and improving access for under-represented populations in Rhode Island.   • Building relationships with community members and leaders to improve community access to the Division of Urogynecology

The RICADV’s Community Engagement Specialist will work to build, strengthen, and nurture the agency’s current and future relationships with member programs, partners, allied organizations, and community groups. Working across all RICADV teams, they will assist in the design and implementation of a community engagement strategy that is both reflective of and responsive to the experiences of those most impacted by domestic violence. Boswell is seeking a contractor (part time) Community Health Worker (CHW) who will serve the Washington County area (South County), and ideally lives in this area. CHW experience and knowledge of local social service resources is a must.

In these times of economic uncertainty, more people are struggling with their social needs – such as having enough money for food and rent. Boswell strives to help them access the healthcare and social service resources that they are eligible. Our goal is to address social needs before they impact people’s health.

As a Community Health Worker (CHW), you will play a critical role in this mission. You will meet with individuals who are going to food pantries, understand what additional health and social concerns they have, connect them with relevant resources, and build a long-term relationship.

 

Responsibilities

  • Visit Boswell’s partner food pantries in person to meet and engage with patients
  • Conduct assessments, offer guidance, provide education, and make referrals
  • Help patients navigate to relevant health and social service resources (e.g. schedule medical appointment, enroll in SNAP), rather than just handing them a list
  • Engage in SMS-based and Telehealth communications with patients for follow-on conversations

  • Continue to build knowledge of social services and relationships within the communities we serve to assist in expanding our partner network

  • Experience: 2+ years of experience assisting people with their social needs in a social service organization or healthcare setting
  • Credentials: CHW certification by the RI Certification Board, or completing the CHW certification within the next 18 months

Job Summary:


The Community Health Worker (CHW) is a peer who has experience in navigating Rhode Island’s health system for themselves, a family member or through previous employment. This CHW will work in RIPIN’s Hospital Care Transition Program, which supports Rhode Islanders who may be good candidates to discharge from the hospital back to their homes or other community settings, but need a little extra assistance to make that possible. The CHW will be a critical part of a comprehensive team providing options counseling, resources and referrals for post-hospital care. CHWs will engage with consumers in hospital settings providing person centered, culturally sensitive support, and building on the values, strengths and preferences of the patient. The CHW will also serve as an effective role model and mentor.

Essential Functions:

  • Assist patients and families in understanding and accessing informal and formal options for post-discharge care benefits including copay and cost of care. Review and educate on benefits and eligibility for Medicaid Fee-For-Service, Medicaid/Medicare Managed Care, Medicare Advantage Plans, and any available private insurances. Assist the consumer in completion and submission of enrollment or benefit applications. Refer consumers to other services and public or private agencies for additional supports as needed.
  • Utilizing motivational interviewing skills and culturally sensitive methods to collaborate with patients to explore preferred post-discharge supports and identify social determinants of health and/or areas of need within their community environment. Review care options including natural supports, home care services, medical equipment, adult day health programs, senior centers and assisted living communities.
  • Coordinate with hospital discharge and health plan staff to enable post-discharge home and community supports to be established in a timely manner.
  • Assist consumers as they transition to independence/case closure by engaging with consumers and providing follow up support
  • Maintain timely, accurate records, documentation, and reports as required
  • Actively participate and complete training and professional development activities
  • Assist in statewide system analysis, planning and coordination with state agencies, state and local boards, community-based organizations, and community rehabilitation programs
  • Accept other duties and responsibilities as assigned

Knowledge, Skills and Abilities:

  • Ability to demonstrate sensitivity towards, relate to, form trusting connections with, and motivate consumers as a peer mentor and to address barriers to care, health and wellness
  • Knowledge of Rhode Island health systems, terminology, supports, and services
  • Demonstrated ability and skill to work collaboratively with co-workers, consumers, families, service providers, and health plans, etc.
  • Skilled and/or willingness to learn and initiate motivational interviewing techniques with consumers
  • Demonstrated prior success in accessing community-based resources in Rhode Island
  • Strong written and oral communication skills
  • Excellent organizational skills to manage multiple priorities and tasks
  • A deep understanding of, commitment to, and ability to carry out the mission, vision, philosophy and values of RIPIN
  • Demonstrated proficiency with Microsoft Office/computer skills to enter data, prepare reports and correspondence
  • Demonstrated written and verbal fluency in Spanish required

Education and Experience:

  • High School diploma or GED
  • Attained or working towards a bachelor’s degree, or a combination of education, experience, and skills to effectively carry out responsibilities and assignments
  • Community Health Workers certification preferred; non-certified incumbents are expected to earn certification within 18 months of hire date
  • Personal experience navigating state and community services and programs on behalf of self or a family member
  • Previous experience supporting families or individuals with special care needs or disabilities or families or individuals accessing health programs and services
  • Demonstrated ability to work both independently and as an effective team member
  • Demonstrated experience working with diverse populations
  • A combination of education and experience demonstrating acquisition of the skills and abilities required

Physical Demands:

While performing the duties of this job, the employee is regularly required to talk or hear. The employee regularly is required to stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls; and reach with hands and arms. While performing the duties of this job, the employee is regularly required to climb stairs, reach, stretch, stand and bend. The employee frequently lifts and/or moves up to 25 pounds. Community Health Workers are required to climb up to three flights of stairs to conduct home and community visits.

Working Conditions/ Work Environment:

  • Primary work location is a climate-controlled indoor hospital or office environment; however, employee will also be required to conduct visits in private homes and various community locations
  • A significant portion of work may be based out of a hospital location, which may bring elevated risk of exposure to COVID-19 or other infectious diseases.
  • Must have suitable space to work remotely at home as needed. 
  • Must be able to provide own reliable transportation to facilitate visits to client’s home or community setting and travel between multiple provider sites.
  • Flexibility for occasional travel related to job requirements
  • Willingness and ability to work limited evenings and weekends as needed
  • Provide own reliable transportation with proof of RI minimum requirements of auto insurance
