Job Summary:

The Medicare-Medicaid Outreach Specialist will work to raise awareness of RIPIN’s program that helps people with Medicare and Medicaid coverage (“duals”) access healthcare and health insurance. The Outreach Specialist will perform community outreach to individuals enrolled in or eligible for Rhode Island’s Medicare-Medicaid Plan (MMP), including low-income older adults and individuals with disabilities. RIPIN program staff assist consumers in obtaining access to needed health care services, and coordinate with health insurance care managers to provide whole-person support for the consumer.

The Medicare-Medicaid Outreach Specialist will develop, organize, and conduct outreach events, with special focus on disadvantaged communities including individuals with developmental disabilities, consumers with serious and persistent mental illness, homeless individuals, and consumers living in institutions or alternative living settings. The Medicare-Medicaid Outreach Specialist will manage and build relationships with outside partners, including community-based organizations, healthcare professionals, residential facilities, State agencies and workgroups, and the MMP.

Essential Functions:

  • • Build a working knowledge of RIPIN’s program supporting Medicare-Medicaid beneficiaries by working with RIPIN staff, in order to help consumers understand and access RIPIN’s services.
  • • Partner with RIPIN colleagues and other stakeholders to develop an engaging outreach and marketing program that educates consumers and enables them to advocate for their needs.
  • • Coordinate with program staff and staff of other RIPIN programs to provide support for health care access issues identified during outreach events.
  • • Provide culturally competent outreach to various target groups, including individuals with developmental disabilities, individuals with serious and persistent mental illness, homeless individuals, homebound individuals, consumers living in institutions, and consumers who lack literacy skills or who have communication impairment.
  • • Develop and conduct events at senior centers, adult day centers, nursing facilities, group homes, and assisted living facilities.
  • • Design and implement marketing activities designed to reach low-income older adults and individuals with disabilities.
  • Manage program relationships with outside partners including community-based organizations, State partners, healthcare providers, and the MMP
  • • Provide an average of eight to ten outreach activities and events per month at locations throughout the State.
  • • Assist in establishing and implementing an outreach evaluation system to pursue continuous outreach quality improvement.
  • • Support RIPIN outreach activities related to other programs and services as needed including serving as a HealthSource RI Navigator.
  • • Accept other duties and responsibilities as assigned.

  • • Preferred minimum of two years’ experience supporting families and individuals with special healthcare needs, OR two years’ experience conducting outreach activities to diverse communities, OR any combination of the two.
  • • Personal experience navigating healthcare, special education, and/or other social support systems and services for self or family member highly desired
  • • Fluency (speaking, reading, writing) in English and Spanish required
  • • Community Health Workers certification preferred; non-certified incumbents are expected to earn certification within 18 months of hire date.

The Bilingual Support Navigator is a peer who has lived experience in navigating Rhode Island’s systems of care for themselves, a family member or through previous employment. The Rhode Island Department of Health (RIDOH) is working to raise awareness and offer resources to healthcare providers around substance use conditions, particularly as they relate to pregnant and postpartum people and their babies and families. RIDOH wants to ensure that healthcare providers are aware of the impact that various substances can have on their pregnant or postpartum patients and have the resources to support them. Substance use and pregnancy 

Annually in Rhode Island, at least 5% of the approximately 10,000 babies born are exposed to substances that may affect their biopsychosocial health, development, or behavior in the short and/or long term. Substances of exposure can include certain prescribed medications, illicit drugs, alcohol, cannabinoids, and tobacco. 

Resources and supports are available  Parent Support Network of Rhode Island (PSNRI) Through PSNRI’s Healing Mother and Baby Program , pregnant people living with a substance use condition can call or text 401-318-9577 to speak with an English or Spanish-speaking peer recovery coach and access free, confidential help, and support.  

RI MomsPRN This is a free psychiatric telephone consultation resource for healthcare providers who treat pregnant and postpartum people. Healthcare providers can call 401-430-2800 or email RIMomsPRN@CareNE.org Monday through Friday, 8 a.m. to 4 p.m. to be connected to perinatal specialists in real-time.

POSITION SUMMARY:   

This is an Office on Violence Against Women-funded position and part of the Disability Grant Program.  This position will work in close partnership with our grant partner, Tri County Community Action.  The Deaf/Hard of Hearing Services Program Manager will oversee all work of this grant project, starting with collaboration, relationship building, and conducting an organizational needs assessment.  The Manager will identify gaps in community-based services, housing, legal assistance, medical care, and other supports for individuals who are Deaf/Hard of Hearing and victims of abuse.  The Manager will serve as the primary author of our action plan and coordinate the other project deliverables, work with a technical assistance advisor, conduct trainings, and submit reports back to our federal funding source.

 PRINCIPAL JOB RESPONSIBILITIES:

  • Plan and conduct a needs assessment for survivors of domestic violence/sexual assault and individuals who are Deaf/Hard of Hearing (HoH), identifying the current strengths and resources that are available for victims who seek assistance and individual who are Deaf/HoH who seek services, as well as identifying the gaps and challenges for survivors who identify as part of this community. 
  • Develop a strategic plan for collaboration comprised of Sojourner House and Tri-County Community Action, to deliver high quality, trauma informed, client centered services state-wide to survivors of domestic violence who are Deaf/Hard of Hearing.
  • Develop measurable tools to assess the needs of victims and survivors in the community including people who are Deaf/Hard of Hearing; including but not limited to focus groups, community feedback sessions and surveys to understand the strengths and limitations as well as the needs in throughout the state of RI.
  • Supervise and assist any volunteers/interns who are working on this project.      
  • To a limited extent, the Program Manager will provide some direct services to victims seeking services from Sojourner House. This may include answering phone calls, conducting intakes of new clients, and providing referrals and information to potential clients.
  • Working with the Finance Director and Executive Director, assist with the development of a program budget and ensure adherence to this budget.
  • Complete data entry and submit necessary reports as required for grant compliance.
  • Oversee and support Sojourner House’s work with clients who are Deaf/Hard of Hearing, making sure that internal systems and external systems are trauma- informed and client-centered for individuals with disabilities.
  • Prepare and deliver community presentations on issues related to domestic violence, sexual assault, trafficking and/or housing when requested.

EDUCATION:

Bachelor’s Degree in social work, psychology or related field required; familiarity with American Sign Language is a plus.

POSITION SUMMARY:

The Community Health Worker (CHW) is a vital member of the backbone agency poised to support the Health Equity Zone, to provide culturally appropriate support and resources to the 02907 community, related to COVID-19 testing, contact tracing, vaccination and service delivery.  The CHW will also assess community needs and assist in distributing goods and services for COVID-19 mitigation, resilience and recovery; compliance with prevention guidelines for community members and businesses; and distribution of PPE. You will represent the agency and work with the HEZ Program Manager to develop and implement initiatives that increase agency visibility within the community. You need to be self-motivated, enthusiastic and community-driven to take on this exciting role. This is a full-time (35 hours per week) position. The typical schedule would be normal business hours from Monday through Friday with some nights and/or weekends required during special projects or events

REPORTING STRUCTURE:

The CHW will report to the HEZ Program Manager. 

ESSENTIAL JOB FUNCTIONS:

  • Work at neighborhood level to understand barriers to health/social wellness, and, where appropriate, barriers to vaccine uptake; uncovering new assets and partnerships; reporting to Teams and collaborating on community solutions;
  • Support community COVID-19 response as necessary including support with vaccine registration, quarantine and isolation supports amongst your target population.
  • Serve as project lead on COVID-19 response, to include, but not limited to PPE coordination, reporting, and project support.
  • Build upon the relationship between CHWs and 02907 HEZ population of focus through intentional outreach and engagement efforts (i.e. canvasing, hosting or tabling at community events).
  • Work with the HEZ Project Coordinator to produce timely and high-quality reports and correspondence as required by project.
  • Participate in COVID-19 testing sites/activities to provide education and support.
  • Participate on the HEZ Community Action team (HEZCAT) and other RI DOH meetings related to HEZ and/or COVID-19 response.
  • Assess client needs and assist with the distribution of goods and services to assist with COVID-19 mitigation, resilience and recovery.
  • Help conduct surveys, focus groups and one-to-one interviews with residents to inform them of best approaches to responding to the COVID-19 pandemic.
  • Work at the neighborhood level to understand barriers to vaccine uptake; uncovering new assets and partnerships; reporting to HEZ Team and collaborating on community solutions.
  • Establish relationships with trusted community leaders to identify and document cultural and structural barriers that interfere with accessing testing, vaccination and adhering to response guidance.
  • Utilize the Unite Us electronic referral platform in both making and documenting referrals and referral status. This platform helps to keep track of case notes and progress towards meeting client’s needs.
  • Primarily focus on being the boots-on-the-ground person tasked with getting information out to the community in disseminating COVID-19 related information, support events and hand out personal protective equipment. 
  • Maintain accurate and accessible reports, files and statistics as required by the initiative.
  • Perform additional responsibilities consistent with initiative needs and other duties as assigned.

QUALIFICATIONS:

  • Community Health Workers certification preferred; non-certified incumbents are expected to earn certification within 6 months of hire date.
  • Fluency (speaking, reading, and writing) in English and at least one other language preferred.
  • One or more years experience in community outreach and engagement.
  • Resident of the 02907 zip code (Providence’s West End, Elmwood, Reservoir Triangle, or South Providence neighborhoods) is preferred.
  • Excellent organizational skills to manage multiple priorities and tasks.
  • Valid driver’s license and reliable transportation are required.
  • Has an extensive network within the 02907 community and relationships with 02907 community leaders.
  • High level of interpersonal skills to engage with residents and community partners.
  • Demonstrated proficiency with Microsoft Office/computer skills to enter data, prepare reports and correspondence.

RIDOH MPOX Vaccination Clinics

June 10 and 17: Free Mpox Vaccination Clinics To Be Held at Community Events in Providence

In the United States, cases of mpox (formerly monkeypox) have declined since peaking in August 2022, but the outbreak is not over. The CDC continues to receive reports of cases and community transmission in the United States and internationally. This month, CDC and local partners have investigated a cluster of mpox cases in the Chicago area.

RIDOH will host mpox vaccination clinics at the following community events in coming weeks. All eligible people are encouraged to get vaccinated or to complete the JYNNEOS two-dose series. The JYNNEOS vaccine is free. There is no out-of-pocket cost for vaccination. People do not need insurance to get the vaccine.

Primary Function

The Community Health Worker is a member of the community health team; an interdisciplinary team comprised of Community Health Workers, Behavioral Health Clinicians and practice-based Nurse Care Managers. Works closely with the core team members to support patients who are dealing with complex medical, behavioral health and/or substance issues as well as social determinants of health, and require a more intensive home and community-based intervention. Job Qualifications and Specifications

High School diploma required with a combination of training and skills to effectively carry out responsibilities and assignments (such as previous experience working with patients in a community-based setting). Must have Certification in Community Health Work, or be working toward certification to be completed within 12 months from date of hire. Associates or Bachelor’s degree in Social Work, Community/Public Health or related health sciences field a plus. Experience working with primary care providers or in other healthcare settings. Experience working with patients regarding managing their health, navigating systems, providing care coordination and health coaching is preferred. The ability to travel to various locations in the state and reliable transportation is required. Summary
