About the Role:
The Elder Shelter Care Coordinator will support men and women over the age of 55 who are unhoused and need shelter and assistance in securing suitable housing, including Assisted Living, Nursing Home Care, or independent living. This role involves providing intake services, benefits support, and vital document assistance to help guests navigate the multi-step process of housing referral and placement. The Care Coordinator will bring compassion for the elderly, experience working with the unhoused population, and a collaborative team approach to this important work.
Duties/Responsibilities:
* Assist shelter guests in obtaining vital documents required for housing applications and Coordinated Entry System (CES) packets.
* Support guests in applying for public benefits, including Medicaid/Medicare, SNAP, GPA, SSI, unemployment, and other applicable programs.
* Make appropriate referrals to external providers such as healthcare, mental health, education, and employment services.
* Facilitate internal referrals with Amos House staff, including the Financial Opportunity Center, Behavioral Health Clinic, and Medical Director.
* Collect escrow savings from individuals and provide written receipts.
* Provide comprehensive case management through assessment, case planning, evaluation of services, and support during transitions to Assisted Living, Nursing Home Care, or independent housing.
* Conduct crisis and housing assessments as needed.
* Serve as a liaison between guests and DHS/HealthSource RI to resolve coverage issues.
* Assist guests in identifying and applying for appropriate housing and support them through all move-in activities.
* Develop and implement safety plans and risk minimization strategies, including home visits and accompaniment to appointments, to ensure successful transitions to housing.
* Manage caseloads based on acuity, program duration, and housing stability, balancing individual needs effectively.
* Record all required information in Clarity (HMIS) and AWARDS promptly and organize and maintain resident case files according to professional standards.
* Bill for services where applicable, including Home Tenancy, Home Stabilization, CPRS, and Emergency Medicaid. Document notes, billing dates, and codes in AWARDS and coordinate service dates in the ADP calendar.
* Perform other duties as assigned.
Requirements:
Must-haves:
* Bachelor’s Degree required, or equivalent extensive experience in case management.
* Experience working with elderly and/or individuals with complex medical/social/emotional needs.
* Must be able to work sensitively and effectively with individuals of diverse educational, socio-economic, and cultural backgrounds, including individuals who are homeless, in recovery, or formerly incarcerated.
* Strong organizational, communication, and written skills.
* Proficiency in Microsoft Office, data entry, and understanding of databases to track outcomes.
* Ability to maintain confidentiality in all aspects of the work environment
Nice-to-Haves:
* Candidates that are bilingual in English and Spanish are encouraged to apply.
Physical Requirements:
* Prolonged periods sitting at a desk and working on a computer.
* Must be able to lift to 15 pounds at times.
