Description

Position Summary: The Community Health Worker provides preventative, HIV/STI testing and care coordination for APRI clients. The staff also supports the development and implementation of the wellness initiatives for clients living with HIV/AIDS and administers Ryan White support services, including intakes and record keeping.
Qualifications:
- Bachelor’s degree preferred.
- Bilingual preferred.
- Complete Community Health Worker Certification within first year.
- Training as a qualified professional test counselor (QPTC) is required. If not trained, willingness and commitment to be trained and conduct testing as part of overall job function.
- Experience with HIV/AIDS programs and services preferred.
- Demonstrated sensitivity to lesbian, gay, bisexual, transgender population, racial & ethnic minorities, and those impacted by HIV/AIDS and substance abuse.
- Ability to travel to various locations as necessary.
- Ability to utilize Microsoft platforms and an Electronic Medical Record.
- Ability to comply with client confidentiality while working in a hybrid environment.
- Possession of a valid driver’s license, reliable transportation, and proof of auto insurance is required.
- Bilingual skills are compensated!!
