East Bay Community Action Program (EBCAP) is seeking a part-time, 20 hours/week, Coordinator for the Rhode to Equity Health Equity Zone (HEZ) in East Providence, RI.

As a member of the East Providence Health Equity Zone (HEZ), collaborate with EBCAP staff and the HEZ Steering Committee to increase community awareness of, and participation in, the “Rhode to Equity” grant project. Support an ongoing community-wide needs assessment by engaging several community stakeholders/residents, assisting in addressing the unique social, economic, and environmental factors of East Providence that are preventing individuals and families from being as healthy as possible. Adhere to all grant requirements, report on progress, and work within the HEZ and community to provide feedback on how to best meet community needs.

Coordinate Diabetes Health Equity program development with the EP HEZ Steering Committee and EBCAP staff, particularly Food Pantry staff, focusing on community outreach to expand program awareness, and serve as the staff representative on relevant EP HEZ and community workgroups.

Develop and maintain partnerships with other community agencies and collaborate to create programming.

In collaboration with Food Pantry staff and nutritionist ensure fresh produce items provided monthly to “Rhode to Equity” participants are high-quality and fit the needs of participants.

Collaborate with EPHEZ staff, EPHEZ Steering Committee and workgroup members, state agencies, other EBCAP departments, and community residents to increase food equity within East Providence.

Minimum Required Education- High school diploma or equivalent.
