Position Summary:

Integrated Healthcare Partners is looking to add a Patient Engagement Specialist to their team. Position compensation range starts at $22.00 to $25.00/Hour with annual bonus of $3,000 plus great benefits!!

The Patient Engagement Specialist will offer intervention by providing team and community-based resources and referrals, system navigation, support, care coordination, and ongoing case management to meet those needs.

Job Responsibilities:

  • Evaluate engagement and risk criteria to identify individuals that could benefit from case management.
  • Build trusting relationships, serve as an advocate and mentor with the goal of empowering the patient to become more independent and self-sufficient.
  • Collaborate with other service providers, helping patients make necessary connections and enable patient independence in service access.
  • Administer required screening tools with patients to assess patient behaviors, social needs, and factors that may increase the risk for health and behavioral health problems. .
  • Maintain timely, accurate records, documentation, data entry, and reports as required.
  • Perform other duties as assigned by supervisor in support of IHP in achieving organization goals.

Experience and Technical Skills:

  • AS or BS degree in Social Work, Public Health or Health Sciences preferred.
  • 1-3 years healthcare experience preferred
  • Ability to work independently and collaborate in a team environment.
  • Excellent customer service, patient facing and internal communication skills.
  • Participation in community and agency functions, workshops and trainings.
  • Strong organizational skills including attention to detail and multi-tasking skills.
  • Demonstrated computer skills with proficiency with MS Office.

Salary Range:

  • This is a non-exempt position with a range of $22.00 – 25.00 per hour
