RI CHW Resource Map

Created and maintained by the Rhode Island Department of Health

The Community Health Worker Resource Map is an essential tool designed to bridge connections between Community Health Workers (CHWs), community-based organizations, state agencies and community members. It was created by the Rhode Island Department of Health in response to consistent feedback from CHWs that they did not know how to locate or learn of other CHWs in Rhode Island.

Click this symbol on the top left of the map to access functionality. You may search for CHWs by location, specialty training or by services offered.  Check out the full instructions for map use here

  • Selecting Services: The left-hand index includes checkboxes that allow you to filter the services you are looking for. Simply check the box next to the service category to update the map with relevant resources.
  • Identifying CHW Locations: Each service category is represented on the map with distinct markers, showing the exact locations of CHWs who specialize in the selected services. Click on any marker to view detailed information about the CHWs, including their contact details and services offered.

How to Use the Map

Detailed Service Categories

  • CHWs in Rhode Island: This dropdown menu provides details on each CHW, including their name, contact info, employment details, certifications, specialty trainings, health and social services provided, and the populations they serve. This section also lists the state agencies and Health Equity Zones (HEZs) each CHW is connected to.
  • Employer: An alphabetical list of CHW employers across the state. Select an organization to access CHW contact information and addresses.
  • Service Categories: Each of the following dropdowns organizes CHWs by their area of specialization and location, offering direct contact information:
    • Employment and Income
    • Childcare and Parenting
    • Working with Older Adults
    • Government and Legal Issues
    • Housing and Food Assistance
    • Chronic Health Conditions (Hypertension, Heart Disease, Diabetes)
    • Reproductive, Maternal, and Child Health
    • Mental Health and Addiction

Searching for Additional Services

  • Additional Services: Utilize the magnifying glass icon to search for services not immediately visible in the index, including arthritis support, respiratory condition management, healthy eating initiatives, physical activity programs, clothing and household support and advancing equity work. Use the key words underlined to search.
  • Populations Served: Utilize the magnifying glass icon to search for CHWs serving the following populations:
    • Children/youth
    • Older adults 
    • Children with disabilities 
    • Adults with disabilities 
    • Immigrant/refugee communities
    • LGBTQ+ individuals
    • Low-income communities
  • Partnerships with State Agencies: Utilize the magnifying glass icon to search for CHWs associated with the following Rhode Island state agencies:

How Do I Get On the Map?

CHWs may complete this survey to submit their information for the map. Once your information is received, it will be loaded in as timely a manner as possible. For questions email Mitchelle Abuna at Mitchelle.Abuna@health.ri.gov.
