East Bay Community Action Program is hiring for a Case Manager, Older Adults and Adults with Disabilities for our 100 Bullocks Point, Riverside, RI location. The Case Manager is responsible for providing person-centered, unbiased care coordination and case management services to aging and disabled (AED) population, as well as to adults living with intellectual and developmental disabilities (I/DD). This role involves assessing client needs, developing Person – Centered care plans care plans, coordinating community services, and ensuring comprehensive documentation. The Case Manager works closely with clients, families, and interdisciplinary teams to promote client well-being, maintain compliance with regulatory guidelines, and ensures that all services are delivered in a respectful, confidential, person – centered and culturally competent manner.
Responsibilities:
Coordinate care and services for aging and disabled (AED), and adults with intellectual and developmental disabilities (I/DD)
Conduct comprehensive assessments of clients’ needs, strengths, and resources.
Develop, implement and monitor person – centered care plans, addressing medical, social, and community service needs.
Provide ongoing case management and respond to clients’ inquiries.
Connect clients to additional services and supports.
Ensure compliance with Conflict – Free Case Management (CFCM) regulatory standards and documentation requirements for Cost Share and Medicaid AED and I/DD clients.
Investigate and address Adult Protective Services (APS) cases, ensuring the safety and well-being of vulnerable clients.
Report abuse, neglect, or exploitation cases to appropriate authorities.
Maintain confidentiality and provide culturally competent, person-centered services.
Qualifications:
Bachelor’s degree from an accredited institution in fields such as Counseling, Education, Gerontology, Human Services, Nursing, Social Work, Psychology, or a related field is preferred.
At least two years of relevant experience working with low-income elders and individuals with disabilities.
Valid driver’s license, automobile insurance, and reliable transportation required.
Willingness to travel throughout Rhode Island.
Knowledge of human behavior and the aging process.
Ability to work effectively with people from diverse cultures and backgrounds in a supportive capacity.
