Company Summary

Progreso Latino was founded in 1977 to serve Rhode Island’s Latino and immigrant communities. Local Hispanic leaders saw that the state’s existing infrastructure did not adequately meet the needs of the rapidly growing Latino immigrant population. A small group of community leaders came together to form a multi-cultural, multi-service agency with a mission of helping to build a stronger, more self-sufficient Latino community. Since then, PL has grown into the state’s largest Latino community organization, serving thousands of people each year through over a dozen core programs and services. We have moved from our original storefront office staffed by a few committed activists to a four-story building in the heart of the city that houses over 50 employees.

We have developed critical programs that serve as models for community agencies across Rhode Island. Progreso Latino’s programs include adult education, early childhood education, senior services, health and wellness, workforce development, youth development, domestic violence intervention and prevention programs, Volunteer Income Tax Assistance, immigration and social services, and emergency food relief. We have developed strong relationships with community partners throughout the state, and have touched the lives of tens of thousands of individuals.

Position Summary:

The Community Health Worker is a central position in Progreso Latino’s Wellness Program designed to connect medical patients and members of the community to social services and wellness education programming. S/he would educate medical professionals/schools so that they can refer patients/students to community resources and assist the grant’s team with delivering health education. S/he would facilitate health education for individuals and groups as well as support peer-educators and volunteers as needed. S/he would also assist in the collection of data and the tracking of program outcomes. S/he will also work with schools to assist with COVID-19 education in order to increase vaccination rates.

Scope of work includes:

▪ Work closely with the Health Equity Coordinator and Wellness team at Progreso Latino and program

partners.

▪ Attend community and partner events as needed and recruit participants/ patients for the program.

▪ Attend regular partner meetings Rhode to Equity (R2E) and Health Equity Zone (HEZ) for Central Falls and

Pawtucket.

▪ Seek out innovations in health care that will promote public health and advance the aims of the project.

▪ Outreach to community members and organizations.

▪ Facilitate culturally –tailored education and training for clinical providers, community members and

patients.

▪ Work with HCP (health care providers) to identify patients with chronic health care needs and social

determinants of health (SDOH).

▪ Assist patients, students and community members in addressing SDOH’s.

▪ Refer and funnel patients to Progreso Latino and its programs.

▪ Assist with tracking program outcomes, collection of data of participant/patient progress.

▪ Assist in the evaluation and dissemination of important findings and program results.

▪ Other duties as assigned

Additional Responsibilities for SUD Behavioral Support Candidates:

▪ Screen, case-manage and refer community members

▪ Organize and facilitate peer-support groups

▪ Coordinate with Behavioral Health Partners
