Job Summary:

The OneCranston Health Equity Zone (HEZ), one of 11 HEZs in Rhode Island, is a resident-driven initiative funded by the Rhode Island Department of Health, with Comprehensive Community Action (CCAP) as our backbone agency. Our goal is to support and equip Cranston community members to collaborate and create projects, places, and programming where residents can healthily live, learn, work, and play.

The Community Health Worker (Behavioral Health, COVID-19 Response) will be responsible for engaging in outreach to Cranston residents that may be experiencing behavioral health needs related to COVID-19, in collaboration with providers and organizations. This focus will encompass, but is not limited to, social-emotional needs as well as issues of grief and loss. This CHW will primarily be working in the community with specific target populations, though will work closely with the One Cranston HEZ team to improve health outcomes and coordinate initiatives focused on behavioral health and COVID-19 response.

This is a grant-funded, full-time position through June 30, 2022, with opportunity for extension. Occasional evenings and weekends required.Requirements

  • Research, understand, and convey impact of COVID-19 on behavioral health
  • Engage in regular community outreach and COVID-19 prevention activities within Cranston community
  • Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers impacting behavioral health
  • Assist residents in accessing behavioral health-related services
  • Establish positive, supportive relationships with residents and provide feedback
  • Continuously expand knowledge and understanding of community resources, services and programs provided
  • Participate as part of working groups centered on topics such as behavioral health, trauma, COVID-19 grief and loss
  • Work collaboratively and effectively within a team.
  • Other duties as assigned.

Required Qualifications:

  • Bilingual/bicultural (Spanish or Khmer) encouraged to apply
  • High School Diploma or GED, some college preferred.
  • Community Health Worker Certified*
  • *Community Health Worker training and certification will be provided at no cost if not a Certified CHW
  • Proficiency in Microsoft Office products (Word, Excel)
  • Must maintain professionalism, be able to practice confidentiality, and have sound interpersonal skills when interacting with others.
