Job Description
We are seeking a passionate individual to join our team as a Community Health Worker Team Lead! This person will be joining a newly formed startup organization, and will manage our CHW outreach programming. This will involve managing a caseload of 20-30 clients, as well as delegating additional clients to other CHW team members as needed. You will be conducting screening calls, scheduling appointments, troubleshooting benefits issues, and communicating logistics with our clients.
If you are a creative, dedicated and passionate advocate, this job is for you! You should be self-driven, and able to problem solve issues as they come up. You should be flexible and creative with your problem solving, as you never know what our clients
are going to be dealing with, and resources change all the time.
Responsibilities:
- Advocacy: working with healthcare providers to secure healthcare, benefits, resources, and supplies to support clients in their path to establishing a successful life.
- Education: teaching clients about the system to help them understand steps to take on their own.
- Management: will be managing a team of 2-3 CHWs (eventually) with potential for more as the company grows, as well as translators as required. This will involve scheduling, reviewing case load, and ensuring proper follow-up provision and task completion.
- Multi-Lingual (ideally): All of our clients will speak other languages and may or may not be capable of speaking English. They may or may not be literate in their own language as well.
- Technology: We utilize several software platforms and virtual resources in order to conduct our work as efficiently as possible.
Qualifications:
- Must be certified as a Community Health Worker in Rhode Island (if you are close to certification, please still reach out – we can help with finalizing the application, but you should at least have the education credits completed)
- Must be willing to be paid on a 1099 (independent contractor) basis
- Must be proficient in technology, including Google Drive, email, online phone systems and more
- Must be able to advocate for clients in difficult situations
