CTC – RI Annual Conference

The Care Transformation Collaborative of Rhode Island (CTC-RI) held its annual conference on “Building Capacity for Comprehensive Primary Care” November 1, at the Crowne Plaza Hotel in Warwick. A number of Community Health Workers participated, from learning about finetuning professional processes and workplace culture on patient-centered care to observing new informatics systems that are helping healthcare providers better address social determinants of health.
