The Family Support Specialist (CHW) supports the Manager of Family Support in activities related to family support, outreach, and education. Each CHW brings a specific area of knowledge (e.g., COVID-related services, Autism Spectrum Disorder, education, cultural and linguistic expertise) to strengthen team capacity and better serve the community.

Essential Job Functions & Responsibilities
Family Support & Care Coordination
- Work with families enrolled in the Community Health Worker program, incorporating COVID-19–related education and activities when appropriate.
- Provide health promotion and coaching for individuals with autism and their families, including:
- Screening and assessment of health-related social needs
- Goal setting and action planning
- On-site observation of living situations
- Providing information, guidance, and coaching
Health System Navigation
- Assist members with:
- Engagement and follow-up in primary care and preventive services
- Adherence to treatment plans and chronic condition self-management
- Identifying Medicaid providers and covered services
- Scheduling and attending medical appointments
- Arranging transportation to medical appointments
- Accessing telehealth services and learning telehealth technology
- Help members identify and access relevant community resources and accompany them when appropriate.
Health Education & Training
- Learn and deliver curriculum to train parents and caregivers on evidence-based strategies for raising a child with autism.
- Ensure all educational materials and trainings are current, culturally responsive, and community-appropriate.
- Participate in interdisciplinary care planning using a person-centered, team-based approach.
- Develop and maintain resource materials for families and staff and report updates to the Manager of Family Support.
Outreach & Community Engagement
- Coordinate and participate in community outreach events, including resource fairs, community activities, and the annual Imagine Walk.
- Attend community meetings (e.g., HEZ projects, Thrive by Five, Early Intervention) to connect with families and professionals serving at-risk populations.
- Collaborate with the team to support the organization’s private Facebook group and other social media initiatives.
Training & Facilitation
- Present Parent-to-Parent, Creating the Connections, COVID Health and Safety, and other trainings to families and community members.
- Co-present trainings to clinicians and educators, offering the parent perspective.
- Train to independently lead family support groups.
Data & Documentation
- Enter and maintain accurate documentation in the Electronic Record Portal.
- Track and report training and service data as required.
Minimum Qualifications
Requirements
- Parent of a child with an Autism Spectrum Disorder or a close relative (preferred).
- Certified Community Health Worker or willingness to complete CHW certification training.
- Bilingual preferred.
- Ability to work a flexible schedule, including some evenings and weekends.
Pay Range
$17.00 – $25.31 per hour
