Since 1969, Meals on Wheels of RI has worked to address the issues of food insecurity and social isolation as risk factors for negative health outcomes amongst homebound older adults. Through nutritional and other programming, the organization works to empower seniors across the state to live healthier, safer, and happier lives as they age with independence.

Summary

Community Health Worker (CHW)works collaboratively with home delivered meal program to identify the health and wellness needs of Meals on Wheels clients.

The CHW will be an important part of a team providing support, care coordination, resources and referrals. This is a full-time hourly position for 35 hours per week; Monday through Friday with the hours of 8:30 am – 4:00 pm.

Essential Duties and Responsibilities

  • Assess clients to identify medical, social, and safety needs
  • Provide nutrition education to identifed MOWRI clients
  • Make referrals to appropriate services and agencies
  • Maintain database/documentation when visits/referrals are made
  • Collaborate with community partners to connect clients with support and services
  • Attend health fairs, community meetings and other outreach activities
  • Participate in training and professional development opportunities
  • Support the activities of day-to-day of the home delivered meal program
  • Provide assistance and serve as back-up to other staff as needed
  • Attend and participate in staff meetings
  • Other duties as assigned

Education and Experience

  • 1-3 years of experience in a nonprofit setting
  • Experience working with older adults
  • Community Health Worker Certification or in process
  • Bilingual/Spanish Speaking, preferred
