PROGRAM: Pawtucket Central Falls Rhode To Equity Initiative

FULL TIME: 40 hours per week

Bilingual English/Spanish

Summary:

The CHW will work with clients and their families residing in Pawtucket and Central Falls as identified by partner primary care practices (pediatric and adult), and community-based partners – to improve their well-being by connecting them to health and community resources. Responsible for engaging clients; implementing Social Determinants of Health and other screening tools; identifying areas of need; ensuring referrals are made to appropriately matched services; and supporting individuals with accurate, up to date COVID-19 infection prevention, testing site and vaccination information. CHW will complete individual needs assessments in a variety of settings, including virtually, on-site in offices, in resident homes, and in other community venues as appropriate. CHW will support the Pawtucket Central Falls Rhode to Equity project initiatives when appropriate.

Qualifications:

  • Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required. 
  • Certified Community Health Worker, or ability to be certified within first 6 months of employment.
  • Bilingual in Spanish.
  • Valid driver’s license and reliable transportation.
  • People from populations disproportionately impacted by COVID-19 residing in Pawtucket or Central Falls are strongly encouraged to apply.
  • Ability to document in and navigate an Electronic Medical Record (EMR).
  • Strong interpersonal and social skills with an ability to collaborate with a variety of individuals.
  • Ability to provide services virtually, in resident’s homes, and community locations.
  • Ability to have a flexible schedule to include evening shifts and occasional weekends.
  • Prior experience in behavioral health and/or case management preferred.

Physical Requirements: This position requires agency and community visits, employees in this position must have the ability to:

  • Travel to and from community locations and agency sites, which could include using walkways, stairs and/or elevators.
  • Ability and willingness to utilize required Personal Protective Equipment as required.
  • Follow FSRI professional dress code.
  • Ability to lift up to 20lbs.
  • Ability to communicate effectively.
