Job Description

Reports to: Associate Director

Purpose:

Information & Referral and Center Service Referral:
As a core service of an Independent Living Center, this program is to provide information on community resources and services to persons with disabilities and their families, services providers, and the general public. Information is provided via telephone, email, and mail or one-to-one to walk-ins to our Center and at community informational events.   

The Information and Referral Coordinator is one of the first contacts to the agency, taking referrals to the agency for direct services, and starting the process of IL service delivery.

Duties:

Respond to new referral calls, covering agency criteria and documenting appropriate information to start the process for service provision.

Respond to requests for disability-related information from individuals with disabilities, family members, and service providers. This may include information on: accessible housing, reasonable accommodations, adaptive equipment, OSCIL programs & services, and community resources. Determine the nature of the calls and provide appropriate referrals using the resources available.

Maintain up-to-date resource materials.

Determine an individual’s need for OSCIL services and complete Request for Service form.

Respond to questions concerning ADA related-information and resources available to individuals to advocate for themselves.

Provide backup for general calls to Center, as needed.

Assist in providing information for the agency’s website, Facebook pages and other social media sites making sure the information is accurate and current, and representative of the IL philosophy.

Responsible for the development of agency quarterly newsletter, i.e. identifying pertinent information to be included, writing articles and assisting with the newsletter layout.

Update mailing list for individuals, service providers and other interested parties to receive print newsletter.

Keep other staff apprised of new resources (programs and services) available to meet the needs of the disabled community.

Participate in community outreach activities including staffing agency exhibits and informational displays.

Network and collaborate with other service providers to share information and resources. May be asked to represent the agency at these meetings to gather and disseminate latest information.

Document all activity on specified forms and track community connections that are facilitated (device exchanges); and provide monthly and yearly reports to Home Access Program Manager as requested.

May be required to provide occasional crisis intervention.

Other duties as indicated by the Associate Director.

Education/Qualifications:

1.      Bachelor’s degree preferred.  Will consider person with Associate of Science Degree, Human Services Field.
2.      Must possess excellent written and verbal communications skills.
3.      Be detail oriented as exhibited in past work experiences.
4.      Exhibit knowledge of community support services.
5.      Be able to work independently and possess skill at setting priority for task accomplishment.
6.      Prior experience working with persons having disability.
7.      Must be willing to travel to local events.
