Job Summary:

The Community Health Worker (CHW) Employer Alliance Project Lead supports the development of a nascent learning community for organizations employing Community Health Workers (CHWs).  The Project Lead will help to build a new network of organizations operating in the CHW sector, discover what they need to thrive, and coordinate and facilitate capacity-building activities of a statewide employer network.  The Project Lead will work in close collaboration with various stakeholders both internally and externally.

Essential Functions:
•    In partnership with Consultant(s), lead the strategic development and implementation of a learning community to help organizations in the CHW sector build the capacity they need to build out effective CHW programming.
•    Develop and maintain strong working relationships with various stakeholders, including community-based organizations in the CHW sector, healthcare providers, healthcare payers, the RI Department of Health, and others 
•    Build consensus, define objectives, and establish protocols, and mechanisms for tracking and monitoring performance measures
•    Organize regular convenings of CHW employer members for training, technical assistance, and network building.
•    Build an online resource library of trainings and reference materials for CHW employers on topics such recruiting and retaining CHWs, supporting CHW certification and recertification, integrating CHWs into clinical settings, CHW sustainability opportunities, CHW billing, CHW supervision, and other topics of interest to the target community of organizations.
•    Research best practice that leads to an innovative and sustainable CHW workforce. Develop work products to inform future cohorts of employers.  
•    Manage a set of core funded partners, including other employers with CHW expertise and outside technical assistance consultants.
•    Develop Objectives and Strategies — Establish long-range objectives and specifying the strategies and actions to achieve them 
•    Coordinate activities to support the training and development of organizations identified 
•    Monitoring Alliance partners to ensure they are in alignment with programmatic goals and deliverables and in compliance with state and federal laws

Qualifications

The Community Health Worker/Peer Recovery Specialist (CHW/PRS) is a diverse and integral position here at Lifespan. The CHW/PRS focuses on addressing Social Determinants of Health and provides patients with individualized support around their specific longitudinal healthcare and social needs. CHWs/PRSs assess patient needs create a plan of care with the patient and implement the plan to help the person reach the identified goals. The CHW/PRS helps with the navigation of social supports including basic needs support housing justice system involvement retrieving vital documents immigration status support substance use treatment and harm reduction referrals and referrals to health providers obtaining health insurance and income transportation and more. CHWs/PRSs are expected to understand the basic principles of chronic disease management so that they can support clients in achieving their health goals as defined in collaboration with a physician or clinical provider. 

Responsibilities:

The applicant should be a dynamic motivated individual with thorough knowledge of local social services and community resources. They should also understand what the Community Health Worker/Peer Specialist frameworks represent how to integrate the frameworks into their direct care work and how to translate these frameworks to care teams in the hospital and in other care settings. Patients being supported by CHW/PRSs are some of the most vulnerable and have a multitude of health issues that need to be addressed. CHW/PRSs should understand Trauma Informed Care and how to utilize tools such as Motivational Interviewing to help the patient identify their needs and support them in the most effective ways.

The applicant should have completed Community Health Worker and/or Peer Recovery Specialist training upon hire or they will need to complete the CHW and/or PRS training within one year of hire.

Specifically the role of the CHW/PRS is to:

Work collaboratively on a large interdisciplinary team while utilizing an integrated client care model in conjunction with best practices.

Engage in effective motivational interviewing street outreach and case management and documentation of all patient encounters.

Facilitation collaboration with the medical home-primary care providers and specialists social services case managers social workers and mental health professionals to deliver interventions that will address social needs and maximize patient health outcomes.

Advocate for patients in community settings and/or with other systems and partner agencies to prevent unnecessary eviction incarceration loss of income utilities shut off and other negative social outcomes.

Use professional judgement when responding to crises and understanding the importance of a team approach to handling traumas experienced by patients. and adhere to established policies to respond to patient crises including but not limited to loss of housing relapse suicidality and medical emergencies.

ESSENTIAL DUTIES:

Under supervision of the CHW/PRS Supervisor and with oversight from the Manager of Discharge Planning the CHW/PRS is responsible for performing the following essential job functions:

Complete initial assessments with the patient and providers to identify specific areas of need patient strengths and barriers to care

Work to build a trusting and supportive relationship through continued rapport building techniques

Help patients develop and maintain systems to manage their care through goal setting and motivational interviewing

Provide patients with education about how to address their personal needs in order to build self-efficacy

Educate patients about substance use disorder treatment options and facilitate initiation of and referral to evidence-based treatment including medications for opioid use disorder. This includes supporting the process of administering buprenorphine in the ED.

Educate patients about their medical diagnoses and support them in connecting with specialists and medical care teams for follow up care.

Referral and navigation to community harm reduction addiction treatment recovery and medical based services

Help to address any logistical barriers scheduling complications childcare needs etc. that would prevent a patient from showing up to their appointments.

Accompany patients to appointments with health care and social services providers when needed to provide support and advocacy.

Coordinate and collaborate with the patient�s care team (existing health care providers case workers social workers etc.). Mobilize existing team members to action on behalf of the patient.

Document care coordination notes in the EMR (electronic medical record).

Follow up with case load weekly.

QUALIFICATIONS – EXPERIENCE:

Two (2) years of verifiable experience providing information education intervention and/or referral services to culturally diverse populations. Recent experience (last 5 years) working as part of a multi-disciplinary team in a health care setting preferred.

  • � Educates patients about the health care system appropriate sites of care and self-navigation all in an effort to help the patient build skills to become self-sufficient and manage their health independently
  • � Documents work with patients through appropriate record keeping; assists with gathering data relevant to program evaluation as appropriate
  • � Participates in team meetings and RIDOH Project Meetings
  • � Serves as liaison between the professional staff and the community including developing relationships with various stakeholders in the community
  • � Attends ongoing training for community health workers
  • � Complies with federal and local confidentiality laws including HIPAA ensuring patient privacy
  • � Adheres to Coastal Medical guidelines and policies for protecting patients’ demographic clinical and financial information
  • � Performs other job-related duties as assigned

Other information:

Education and Experience:

  • � High school diploma or equivalent
  • � Two (2) years of experience providing information education intervention and/or referral services to culturally diverse populations
  • � Additional training in the medical field with a social service background or demonstrated possession of the competencies necessary to perform the work
  • � Community Health Worker or Peer Recovery Specialist certification in Rhode Island
  • o Certification may be obtained within one year of employment
  • � Any combination of education and experience that is substantially equivalent is also acceptable

  • � Proficiency in the use of an EMR and knowledge of medical terminology
  • � Ability to handle high volume and diverse assignments
  • � Capacity to multi-task think critically and problem solve
  • � Clear assessment creativity judgment and decision-making skills
  • � Ability to take initiative organize prioritize and follow through with work assignments assuring fulfillment of plans and goals
  • � Excellent interpersonal skills and ability to work with and through others to achieve results
  • � Excellent written and verbal communication skills
  • � Must handle the most sensitive and confidential matters with the utmost discretion
  • � Proficient with technology and use of Microsoft Word Excel Outlook and Web-based applications

Physical Requirements

  • � Prolonged periods of sitting at a desk or standing while using a computer
  • � Work and move among all departments of Coastal Medical group
  • � Excellent hand/eye coordination and manual dexterity to operate a personal computer or laptop as well as standard office machines and equipment as it pertains to one�s position
  • � Lift up to 15 pounds
  • � State-wide travel and operation of personal motor vehicle
  • o Must have a valid driver�s license clear driving record and proof of auto insurance

Visual Acuity Hearing and Speaking

  • � Capability to transfer information from original source to paper computer telephone and in person
