Position Summary: The CHW will work with children, adolescents and their families primarily in Providence, Pawtucket and Central Falls. The CHW will screen families for Social Determinant of Health (SDOH) barriers, and connecting them to health and community resources as appropriate. This role is responsible for engaging clients; implementing Social Determinants of Health and other screening tools; identifying areas of need; and ensuring referrals are made to appropriately matched services. CHW will complete individual needs assessments in a variety of settings, including schools, pediatrician offices, in family homes, and in other community venues as appropriate. The CHW will work collaboratively across FSRI and external community-based programs and initiatives as needed.


Qualifications: 

  • Associates or Bachelor’s degree preferred, High School Diploma or Equivalent required.
  • Certified Community Health Worker, or ability to be certified within first 6 months of employment.
  • Experience working with the childhood and adolescent population.
  • Bilingual in Spanish required.
  • Valid driver’s license and reliable transportation.

Consumer Resource Specialist

Job Summary:

The Consumer Resource Specialist will provide comprehensive phone and in-person supports to families, individuals, and school systems navigating health, education, and special education systems as relates to the Individuals with Disabilities Education Act (IDEA) and state/federal insurance regulations. The Consumer Resource Specialist will ensure families and individuals are supported in a responsive and timely manner with a primary focus of providing consumers and their families with information, resources, guidance, and assistance using a peer-to-peer support model.

Essential Functions:

  • Provide daily telephone support to families, community members, and consumers regarding health and/or education/special education matters, offering information, resources, support, and problem-solving to assist families in working together with private and public entities. Provide wraparound case management and advocacy to consumers with complex needs
  • Assist consumers in understanding and accessing informal and formal dispute resolution options and procedural safeguards in health and education. Document any grievances: assist in the coordination and submission of grievances as required by funding sources and/or other public and private agencies. Refer consumers to other services or public or private agencies for additional supports as needed
  • Collaborate with RIPIN staff to continuously develop, assess, and implement outreach plan for call center programs to insure seamless service delivery and to avoid duplication of services. Continuously develop, assess, and implement program awareness campaign to generate consumer utilization of call center resources

Qualifications

Knowledge, Skills and Abilities:

  • Advanced knowledge with the Individuals with Disabilities Education Act (IDEA) required

Job Summary:

The Family Voices Program Lead works closely with the Policy, Advocacy, and Family Voices Manager to oversee the day-to-day functions and performance of the Family Voices program. The Program Lead also directly supervises a small group of staff and serves as an effective mentor, coach, teacher, and resource, providing regular feedback in the form of one-on-one biweekly meetings with assigned staff. Along with the Manager, the Program Lead serves as a liaison between RIPIN and partner agencies and represents RIPIN at local and state meetings and committees. Essential Functions:

  • Support the Policy, Advocacy, and Family Voices Manager with oversight and coordination of the day-to-day activities of the Family Voices department, including managing multiple projects, ensuring timely and high quality completion of grant deliverables and reports, and supervising staff
  • Provide direct supervision to Community Health Workers, including recruiting, hiring, oversight through regular one-on-one meetings and evaluations of staff performance, support with caseloads as appropriate and documentation
  • Ensure programs and services provided are relevant, coordinated, of high quality, meet member needs, are in compliance with RIPIN standards and meet standards/needs of contractors/funders
  • With support from the Policy, Advocacy, and Family Voices Manager, coordinate the day-to-day functions and performance of the Family Voices program, including RIPIN’s Family-to-Family Health Information Center, convening and facilitating support groups, maintaining the RI Medical Home Portal, soliciting feedback from community members at focus groups and listening sessions, educating and empowering the communities RIPIN serves, convening the Family Voices Leadership Team, and more

Job Summary:

The Bilingual RIPIN Early Intervention Parent Consultant Program is funded through the Executive Offices of Health and Human Services, the lead agency for Rhode Island’s Early Intervention system. RIPIN oversees the hiring, training, and placement of a parent consultant in each of the Early Intervention Programs, located throughout the state. The primary purpose is to provide the Early Intervention system with an authentic parent voice and to support, educate, and inform families whose children are enrolled in an Early Intervention Program (EIP).

Qualifications:

Description:

The Community Health Worker – Case Management builds trusting and supportive relationships in order to coordinate care for patients with complex chronic conditions. Duties and Responsibilities:

  • Must be comfortable completing visits independently or as a care team in members’ homes. Completes home visits to perform assessments on member’s basic social and human needs.
  • Dependability and time flexibility, when necessary; commits to working across all interdisciplinary teams, educate members on alternative to Emergency Department (ED) for non-emergent care; assist, train, and coach members to obtain, schedule and reschedule health care appointments; explore transportation options and provide tips and techniques to ensure follow-up with service providers.
  • Complete hospital or skilled nursing facility visits to support transitional care management process for community resource needs.
  • Educate and help to keep members in compliance with their care plan goals and better utilizers of the health care system
  • Coordinate the provision of services/care to meet the member’s social and health needs. Aids members, families, and caregivers in accessing basic resources such as food and housing
  • Collaborate with other health professionals, including contacting primary care provider offices, care management staff and community social service agencies on behalf of the member.
  • Prepare and participate in team meetings

Job Description

Primary Purpose:

The CHW Wellness Coordinator plays a critical role in establishing Rhode Island College (RIC) and the Community Health Worker Association of Rhode Island (CHWARI) as the higher education and training partners respectively. The purpose of this position is to strengthen community health work delivery through the recruitment, training, and apprenticeships of Community Health Workers.

Essential Duties:

The CHW Wellness Coordinator will serve as the liaison between RIC, CHWARI, and community-based organizations by providing support, supervision, data collection, and communication among practice sites, and newly trained CHWs. This position will require special attention to end consumer, impact on the employing organizations, CHW student, and grant deliverables. CHW Wellness Coordinators will also provide coaching, mentoring, and training for CHW student apprentices through monthly learning collaboratives. One on one support for CHWs as needed in their day-to-day activities, in attending to their student and apprenticeship duties, assistance in overcoming barriers to accomplishing them, with guidance through in person, telephone, and video conferencing. Monitoring and tracking student progress and completion of the trainings, and apprentices’ progression through the apprenticeship milestones and demonstration of competencies. Provide oversite and support for meetings with grant partners and evaluation guidance.

Staff will report to Project Director and will be responsible for the following:

  • Observe, communicate with, and support CHW trainees and apprentices.
  • Responsible for monitoring team activities, required trainings, data collection, and data entry related to outcomes.
  • Participate in weekly team meetings held in person and virtual.
  • Monitor and inform Project Director and partners of system issues related care coordination for end consumers, organizational needs, care continuum obstacles due to burnout, discrimination, absence of services/resources, communication obstacles.

Responsibilities:

  • The CHW Wellness Coordinator will establish CHWARI as the CHW support center through creation of a pipeline of recruitment, training, and apprenticeships of Community Health Workers.
  • Represents and instills the values embodied by CHWARI, empowering people of the community to focus on advocacy and access to healthcare, and SDOH within the underserved populations of their communities.
  • Connection between CHWARI, RIC and community sites, onsite supervision and supporting of CHW apprentices, grant meetings and evaluation of CHW progress.
  • Supervision, data collection, and communication among practice sites, mentors, CHWs and RIC with attention to end consumer, organizational, student and grant needs.
  • Providing culturally competent coaching, mentoring, and training through one and one and monthly learning collaboratives, communicating with onsite mentors on progress of apprenticeship, providing solution-based guidance to apprentices and mentors as needed.
  • Monitor and track apprentice progress and completion of mandatory trainings
  • Monitor and track apprentices’ progression through the apprenticeship milestones, demonstration of on the job learning competencies, and stipend disbursement
  • Provide support for new CHWs as they submit for CCHW certification through the RICB
  • Participate in meetings with employer partners, CHWARI, and RIC.
  • Communicate regularly with Program Director and partners on system issues related to care coordination for consumers, student-organizational conflicts, CHW obstacles due to funding, discrimination, absence of services/resources, communication obstacles

Required Skills, Knowledge, and Abilities:

Experienced CHW in maintaining partnerships between community-based organizations and healthcare organizations. Ability to plan, organize, and implement solutions for increasing access to care in community settings. Ability to work in situations requiring empathy, tact and collegiality. Knowledgeable about community health, organizations within the community, resources available and how to access them. Ability to adapt to change with a positive attitude. Ability to listen and communicate effectively in verbal and written form. Good technology and digital literacy skills including MS Office, Google Suite, Zoom, and general computer knowledge.

Qualifications

  • Previous experience in community health work, and navigation of resources most useful to CHWs as they work within communities of vulnerable populations.
  • Understand the mentor/mentee relationship and what it means to support these roles in the workplace through education, training, and experience.

  • Skills required to mentor and provide support to CHW trainees through certification and apprenticeship timeline.
  • Understanding and empathizing with the challenges CHWs face entering the workforce. Having the ability to provide guidance to new CHWs faced with assisting and overcoming barriers to health of individuals served.
  • Promote participant learning and achievement through coursework, on the job training, mentorship, and support leading to retention of CHW workforce.
  • Ability to multitask, follow direction, and responsiveness in timely manner
  • Familiarity with rules, regulations, and ethical practices most commonly affecting CHWs.
  • Create and instill change with a positive outlook and attitude.
  • Experience with community health work settings and supervision; building repoire between community-based organizations, higher education; and supporting program development through these partnerships

About the Organization and Funding:

The Community Health Worker Association of RI is housed at the Institute for Education in Healthcare (IEH) which is situated within the School of Business at Rhode Island College (RIC). The IEH was formed in 2016 as a unifying body tasked with building partnerships across higher education, health and behavioral healthcare industry partners, community partners, and consumer groups for the purpose of identifying and responding to workforce needs across the care continuum. The IEH received a $2,300,000 dollar (over four years) award from the Health Resources and Services Administration (HRSA) for strengthening communities hardest hit during the COVID-19 pandemic by fortifying the Community Health Worker (CHW) workforce. Through the grant IEH aims to train and deploy 225 CHWs through apprenticeships/mentorship.

ABOUT US: West Elmwood Housing Development Corporation (WEHDC) serves as the backbone agency of the 02907 Health Equity Zone (HEZ). A Health Equity Zone (HEZ) is an innovative, place-based approach that brings people together to build healthy, resilient communities across Rhode Island.

POSITION SUMMARY: This Community Health Worker (CHW) is an entry level position. The CHW is a vital member who will apply their deep understanding and experience, language and/or culture of the community they serve to carry out one or more of the following roles: Provide culturally appropriate health education, information, and outreach in community-based settings, such as homes, schools, local businesses, and community centers. Serve as a bridge between individuals, communities and health and human services, including actively building individual and community capacity in 02907. Advocate and assist with 02907 resident and community needs. Work alongside HEZ and Rhode 2 Equity team initiatives to increase agency visibility within the community including distributing goods and services for COVID-19 mitigation, resilience and recovery.

This is a full-time (35 hours per week) position. Work will take place during normal business hours from Monday through Friday 9:00am-4:30pm with occasional evenings and/or weekend required during special projects or events. Sign on Bonus of $1,500.

REPORTING STRUCTURE: The CHW will report to the HEZ Program Manager.

ESSENTIAL JOB FUNCTIONS:

  • Complete administrative tasks on an ongoing basis to ensure proper and organized record keeping, working with staff members when/if necessary.
  • You need to be self-motivated, enthusiastic and community-driven to take on this exciting role.
  • Build upon the relationship between CHW and 02907 population through intentional outreach and engagement efforts (i.e. door knocking, hosting and/or tabling at community events, etc).
  • Primarily focus on being the boots-on-the-ground person tasked with getting information out to the community in disseminating COVID-19 related information, support events and hand out personal protective equipment.
  • Perform additional responsibilities consistent with initiative needs and other duties as assigned.

QUALIFICATIONS:

  • You need to be self-motivated, enthusiastic and community-driven to take on this exciting role.
  • One or more years’ experience in community outreach and engagement a must.
  • Willing to complete Community Health Workers certification training within 6 months of hire date.
  • Fluency (speaking, reading, and writing) in English and at least one other language preferred.
  • Excellent organizational skills to manage multiple priorities and tasks.
  • Valid driver’s license and reliable transportation, preferred.
