East Bay Community Action Program (EBCAP) is seeking a full time Peer Specialist for the Behavioral Health Services Division at the 2 Old County Road location in Barrington, RI. Join our expanding Behavioral Health Services Division in support of a dynamic multidisciplinary team. Utilizing training and personal knowledge/experience of recovery, serve as a role-model for new and existing clients within the Health Home treatment environment, as well as assisting peers in making behavioral and/or physical changes leading to positive lifestyle improvement

The Peer Specialist requires a Peer Specialist and/or Wellness Coach Certification obtained through the State of Rhode Island, as well as reliable transportation, valid driver’s license and automobile insurance. This position is Monday through Friday with periodic rotation of an early evening, Saturday, Sunday, and/or Sunday. The successful candidate would receive training in EBCAP’s electronic medical record which is NextGen.

The Candidate must utilize his/her personal knowledge/experience of recovery to be a role-model for new and existing clients. He/she will assist clients in recognizing strengths and weaknesses, provide the tools for clients to learn coping skills, role-model self-recovery, and provide coaching and mentoring with the goal of client self-sufficiency and recovery. Hours are 8:00 AM to 4:30 PM four days a week and 10:00 AM to 6:30 PM one of the remaining days of the week. The position requires one Saturday a month and a periodic holiday during the year to accompany a Community Treatment Specialist on client case management home visits.

Job Summary:  

The Health Education Specialist is an integral part of the Community Health Network (CHN) and the Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN) program. The Health Education Specialist engages with individuals telephonically using tools such as Motivational Interviewing to connect individuals to CHN and WISEWOMAN programs and services, identifies and addresses barriers to participation, and provides follow-up to providers.

Essential Functions:

  • Assess needs of individuals referred to Community Health Network and/or WISEWOMAN, provide support and health coaching to participants to achieve their healthy lifestyle goals, and make referrals to RIPIN programs and/or other community-based organizations as needed to assist individuals.
  • Utilizing Salesforce and other Electronic Health Record systems to process referrals and provide detailed documentation of consumer interactions per RIPIN and Rhode Island Department of Health (RIDOH) business practices.
  • Maintain relationships with community health centers and community organizations to identify community resources that support health and wellness (e.g., bike paths, walking clubs).
  • Communicate with referring primary care providers and community-based organizations to follow-up on referrals and provide updates on participation as needed.
  • Maintain confidentiality standards and accurate documentation of case records; maintain complete and accurate records within agency and program guidelines, including guidelines set by RIDOH and RIPIN.

Job Summary:

The Bilingual Peer Support Coordinator – Special Education, will provide comprehensive supports and a continuum of services for families as it relates to the Individuals with Disabilities Education Act (IDEA). Educates patients about the health care system appropriate sites of care and self-navigation all in an effort to help the patient build skills to become self-sufficient and manage their health independently.

Documents work with patients through appropriate record keeping that follows the clinic�s policies and procedures; assists with gathering data relevant to program evaluation as appropriate.

Operates effectively in a multi-disciplinary clinical setting which may include participating regularly in clinic program meetings and attending patient medical visits upon request.

Participates in team meetings and assists as needed in any activities related to the clinic or the Brown University Health Community Health Institute.

Serves as liaison between the professional staff and the community; including developing relationships with various stakeholders in the community.

May transport ambulatory patients between their homes and clinics hospitals or other social agencies and meet with patients at home in the hospital or in community settings.

Attends ongoing training for community health workers.

Practices clear effective consistent communication with clients and colleagues.

Demonstrate privacy and confidentiality meeting all related training and practice requirements of the Brown University Health system.

Works nights and weekends as required.

QUALIFICATIONS – EXPERIENCE:

Two (2) years of verifiable experience providing information education intervention and/or referral services to culturally diverse populations. Recent experience (last 5 years) working as part of a multi-disciplinary team in a health care setting preferred.

Community Health Worker certification in Rhode Island preferred.

Experience with motivational interviewing advising/counseling clients and/or participating in health promotion and health education activities.

Excellent verbal and written communication skills.

Excellent organization thorough record-keeping follow through and ability to juggle multiple priorities in fast-paced environment with multiple collaborators.

Demonstrated excellent attendance and reliability.

Prior experience using an electronic health record preferred.

Bilingual English/Spanish preferred.

Job Overview:

As a community outreach consultant, you will support the Rhode Island Public Association program coordinator with grant-related community outreach efforts. This is mainly a remote position, but it will involve travel to community events.

The community outreach consultant plays a vital role in promoting public health by collaborating on the planning and execution of community outreach and engagement initiatives.

Primary Responsibilities:

The position entails acting as a liaison with our partnering community organizations, actively engaging with them by participating in meetings and events, identifying opportunities for collaboration, and raising awareness about public health issues.

Other Duties

In addition, this position will assist RIPHA’s Project Coordinator with assigned tasks, such as:

  • Designing and disseminating health promotion materials.
  • Collaborating on the planning and execution of community outreach and engagement initiatives
  • Attending monthly and quarterly community meetings and identifying new ways to collaborate.
  • Coordinating and developing strategies for volunteer engagement during RIPHA’s conference in April 2025.
  • Developing strategies to disseminate health promotion materials among community partners and high-risk communities.
  • Facilitating community engagement for community assessments, including focus groups and community discussions about specific health topics.
  • Partner with and support the program coordinator by completing delegated tasks.
  • Overseeing the engagement and coordination of the RIPHA ambassador program and Health Equity content evaluators.
