Position Summary: The Community Outreach Coordinator works to increase access to SNAP and other federal and state social service programs through outreach and direct application assistance to underserved populations in Rhode Island.  The Community Outreach Coordinator finds innovative ways to increase the outreach capacity of our member agencies, providing technical assistance, including educational opportunities about state and federal assistance programs.  The person in this position provides program training and assistance to staff and volunteers of our member agencies and works as a team member to achieve the goals and objectives of the department and carry out the mission of the Food Bank.

Duties and Responsibilities:

1.    Responsible for understanding local, state, and federal assistance programs for low-income individuals and families, including eligibility requirements and how to access programs. 

2.    Direct outreach, including:

a.    Conduct outreach work at Food Bank agencies and educate low-income individuals about SNAP benefits, administer pre-screenings, assist with application completion and follow-up.

b.    Educate staff and volunteers of Food Bank member agencies about ways to provide SNAP outreach and assistance at their food assistance program sites.   

c.    Develops and conducts outreach modules that address existing program barriers and provides direct assistance to guests, member agencies, and community partners.

d.    Delivers presentations on state and federal programs, as assigned.  Develops written materials, videos, displays, etc. following company and department branding.

3.    Provides information and quality referrals based upon a comprehensive understanding of food resources and other state food assistance programs. 

4.    Connects member agencies and their program guests with existing services through monthly resources. weekly eblast submissions, and other communication tools.

5.    Advocates to reduce barriers that hinder SNAP enrollment and attends state SNAP Advisory meetings.  Supports Food Bank legislative advocacy initiatives, as assigned.

6.    Participates in community meetings, collaborations, and forums to educate groups about state and federal programs, as assigned.

7.    Collaborates outreach efforts with Healthy Habits, Kids Cafe, and Agency Programs.

8.    Meets annual and monthly program metrics and collects data and produces reports for program development for the Food Bank, as assigned.

9.    Other duties as assigned.

Skills and Qualifications:

    A bachelor’s degree or three years related work experience or the equivalent.

    Three or more years of experience working in or with social service organizations.

    Bi-lingual verbal and written Spanish required.

    Must be able to pass a criminal background check.

    Experience in community outreach and advocacy.

    Ability to work with people from diverse social and ethnic backgrounds.

    Ability to communicate effectively when speaking and in writing using standard forms of professional and office communication.

    Effective problem-solving skills and the ability to assess issues and develop new strategies.

    High level facilitation skills and experience in customer service.

    High proficiency with Microsoft Windows and Office environment (Word, Excel, Outlook) and standard office equipment.

    Ability to manage multiple projects with attention to detail, deal with interruptions, and maintain focus on tasks while producing accurate work.

    Ability to make presentations and develop and deliver reports to a varied audience.

    Ability to work independently and as part of a team.

Position Summary:

The Community Health Worker (CHW) is a member of an interdisciplinary team comprised of Community Health Workers, Behavioral Health Clinicians, Nurse Care Managers and Medical providers. The CHW works closely with the core team members to support patients who are dealing with complex medical, behavioral health and/or substance issues as well as social determinants of health and require a more intensive home and community-based intervention. CHW visits patients in their homes and in the communities in which they live, providing culturally sensitive approaches to health information to improve health literacy. The Community Health Worker facilitates the patient’s decision-making and self-management to help patients engage in their overall health and achieve their health goals. This individual will be responsible for tracking patient-related activities, monitoring and documenting progress. This position works collaboratively with the primary care team to promote patient-centered care and actively participates in multidisciplinary patient-centered team huddles. The CHW has frequent contact with many community agencies on behalf of the patients served, networking and collaborating on resource identification to improve the overall health of the population. 

Essential Duties include:

  • Helping individuals, families, groups, and communities develop their capacity and access to resources, including health insurance, food, housing, quality care and health information
  • Support individuals with housing needs, providing assistance with applications, case management of those in housing stabilization programs, and coordinate transitions into sustainable housing
  • Facilitating communication and client empowerment in interactions with health care/social service systems
  • Initiates outreach and successfully engages with patients, scheduling appointments and providing follow-up contact within designated timeframes
  • Completes initial intake, including a comprehensive assessment of social needs, functional assessment of the patient in the home setting, and condition of the home if needed within designated timeframes
  • Builds trusting relationship and serves as an advocate and mentor with the goal of empowering the patient to become more independent and self-sufficient
  • Collaborates with nurse care managers, physicians, other care team members (Medical Assistants, Nurses, dental, behavioral health, etc.), hospitals, partner agencies, to improve patient care
  • Work collaboratively in line with Health Equity Zone goals to positively impact populations in Washington County who are identified as at-risk
  • Following WRH policies, obtain releases, gathers PHI from outside providers involved in patient care as needed and processes documents according to program requirements
  • Within scope of CHW training, accompanies patients to doctor’s appointments, assists with food planning/shopping, completing forms for benefit applications, and assists with other tasks as needed that support their medical, behavioral health and social needs
  • Educates patients on appropriate Urgent Visit and ED use
  • Utilizes a multi-disciplinary team approach to address opportunities to plan and coordinate care
  • Utilizes Motivational Interviewing skills and other patient engagement techniques with patients and caregivers
  • Assists in the development of a patient care plan to include actions designed to improve the patient’s health status and remove the barriers that are preventing them from gaining access to high quality and timely primary/specialist care
  • Leverages EMR/chronic disease registry reporting to prioritize patient follow-up
  • Identifies and utilize culturally sensitive approaches and community resources
  • Documents activities and communications in the patient chart
  • Provides training to other team or practice staff as needed
  • Acts as liaison to health plans, hospital, long-term care, BH specialists and home health representatives
  • Works with HEZ partner organizations to improve transition of care and prevent avoidable ED visits for behavioral health
  • Attends required training and collaborative sessions as scheduled
  • Delivering health information using culturally appropriate terms and concepts
  • Linking people to health care/social service resources
  • Providing informal counseling, support, and follow-up
  • Advocating for local health needs through meetings with patients over the phone or in person through community and home visits
  • Providing health services, such as monitoring blood pressure and providing first aid within the scope of training
  • Outreach patients in a timely manner, conduct home or community visits, and administer assessments to identify patient needs
  • Maintain timely, accurate records, documentation, and reports as required
  • Off-site travel will be required
  • Maintain strict confidentiality in all matters
  • Operate within the scope of the Health Information Portability and Accountability Act to safeguard the privacy of protected patient health information
  • Enliven and support the mission, vision, and values of Wood River Health

Our year-round Leadership Transformation Academy (LTA) makes space for youth voice in legislation and policymaking to positively affect our schools, our state, and our world. Position Summary

Young Voices is searching for an energetic and creative Program Coordinator to co-design and facilitate our afterschool program offerings in downtown Providence. The ideal candidate will demonstrate a broad knowledge of current social justice issues and work collaboratively as a member of a five-person team to develop our year-round youth leadership curriculum. The Program Coordinator will provide support for building and maintaining community partnerships and regularly track progress to ensure all deliverables are being met in a timely manner.

Responsibilities

Collaborate with the program team and youth to design, develop, and implement programming that builds authentic youth leadership opportunities within Young Voices.

Co-facilitate biweekly Leadership Transformation Academy (LTA) at our main site in downtown Providence.

Support youth to build skills in capacity building, power analysis, and direct advocacy for all Young Voices campaigns.

Input data including daily attendance, student information, and youth goal sheets.

Participate in weekly planning and evaluation meetings with the program team.

Co-design and facilitate the Summer Workforce Program with the program team.

Lead and support recruitment and retention efforts for all Young Voices programs.

Maintain and expand collaborative relationships with community partner organizations

Support all coalition-based policy projects with partner organizations.

Provide support for all Young Voices fundraising and special event efforts.

Maintain a positive work environment among staff and youth, practicing near-peer role modeling, reliability, and professionalism.

Commitment and Expectations

This is a primarily in-person position with some flexible WFH opportunities, conditional upon approval from the Program Manager.

Business hours are from Monday through Friday, 10AM – 6 PM. Afterschool programming occurs between 3:30PM – 5:30PM. All Program Coordinators are expected to be present in-person at least one hour before the start of program time.

Complete all required training and background checks as requested by partners.

Maintain active First Aid and CPR certifications.

Some required work on late nights, weekends, and overnights with ample notice.

Periodically transport youth to and from program space using company-owned vehicle.

Qualifications

Experience working with BIPOC youth between the ages of 13 and 19.

Experience with the design and delivery of leadership and/or advocacy workshops.

Experience with youth organizing and knowledge of the postsecondary landscape.

Bilingual abilities (Spanish, Yoruba, Haitian Creole) a plus.

Excellent time management and organization skills.

Valid driver’s license and proof of insurance.

Protect Medicaid Resources

“This is NOT a drill — if you haven’t activated your networks yet, please do so IMMEDIATELY! This includes your work networks, coalition partners, parent networks, community orgs, health care providers, health centers, hospitals and everyone who depends on Medicaid. Rope in your personal networks — family, friends, etc. via social media. We need LOTS of calls to Congress ASAP!!!!!” – the words of on of our partners.

As you know, the House Budget Committee has approved a budget resolution that includes a minimum of $880 Billion in cuts to Medicaid. The House is on recess this week but we expect that the budget resolution will be voted on next week. While Trump said Medicaid wouldn’t be cut, today he endorsed the House’s budget resolution bill. So we need to make a concerted effort NOW to stop the House from passing this resolution. Once the House & Senate agree on a joint budget resolution, the relevant committees must come up with the cuts. Members will be under a lot of pressure to vote for the budget resolution to keep the process moving. But if the resolution includes $880 Billion in cuts, it will be much more difficult to stop farther down the line.

  • Use this call-in line to reach your member of Congress and say you oppose all Medicaid cuts: 866-426-2631. (Thank you to SEIU for providing the call-in number; it will remain open for the fight ahead!) Position Summary: The Green & Healthy Homes Initiative seeks an Environmental Assessor professional to perform environmental assessments and housing inspections of residential homes in Providence, RI. This key position aims to help the organization reduce home-based environmental health hazards in clients’ homes. The goal of GHHI is to utilize a single stream housing assessment and intervention model to create safe homes for children, families and older adults that are free of asthma triggers, lead hazards, and safety hazards, and that are weatherized and energy efficient. Join a team that is making a difference in the lives of families and seniors.

The Environmental Assessor will conduct environmental assessments using GHHI’s Expanded Comprehensive Health and Safety Housing Assessment in housing units occupied by children with poorly controlled asthma and in units that have been deferred from local weatherization programs due to the presence of health and safety hazards in the home. Based on the results of the assessment, the Assessor will develop and manage the delivery of scopes of work by contractors to remove asthma triggers and other health hazards from the unit and to the extent feasible, work with collaborating partners to ensure that units are energy efficient. The GHHI Environmental Assessor will work closely with complimentary programs and services such as the City of Providence’s Lead Safe program and the Community Action Partnership of Providence’s weatherization and energy efficiency programs, and other housing programs to ensure maximum impact on health and housing quality for participating families. The Assessor will provide all contractor oversight and will conduct quality control inspections.

Key Responsibilities: 

  • Conduct environmental assessments and housing inspections utilizing GHHI’s Comprehensive Health and Safety Housing Assessment.
  • Develop of Scopes of Work and cost estimates for healthy homes interventions (lead hazard reduction, mold/moisture remediation, integrated pest management, asthma trigger reduction, indoor air quality, household injury prevention, removal of know and tube wiring, etc.)
  • Work with the Program Manager to recruit and retain a network of contractors to perform health and safety interventions
  • Assign work to and coordinate independent contractors, monitor project progress and budgets, track contractor performance and ensure work is being completed according to HUD and EPA regulations.
  • Conduct ongoing site inspections and clearance inspections upon completion to determine whether the unit is safe and contains no remaining health hazards, in compliance with EPA and HUD standards.
  • Work closely with partnering agencies to align healthy housing work with energy efficiency, weatherization, and housing rehabilitation interventions.
  • Coordinate and collaborate with Providence based GHHI team members and partners including the Vice President of State and Local Strategic Services, the Healthy Providence Healthy Homes Project Director, Client Intake Specialist, and partnering community health workers and health care providers.
  • Coordinate and collaborate with GHHI staff conducting similar work in sites around the country to support the development and sharing of best practices and continual learning.
  • Work alongside community health workers to provide information to families on risk awareness, prevention techniques, and prevention resources relating to asthma triggers and health and safety hazards identified in the home during the housing assessment.
  • Install basic safety measures to reduce household safety hazards in participating properties where applicable.
  • Perform initial integrated pest management in participating properties where applicable.
  • Report data in compliance with standards and regulations and maintain responsibility for project reports.
  • Maintain electronic property files and report data in compliance with Program standards and requirements and maintain responsibility for project reporting in GHHI’s Efforts to Outcomes (ETO) client database.
  • Monitor achievement toward program goals and make recommendations towards overcoming barriers for service delivery.
  • Support building relationships with community partners to increase program referrals, build program partnerships and recommend additional resources that program clients can utilize.
  • Participate in weekly and other team meetings as appropriate.
  • Complete expenditure reports and support the development of progress reports.
  • Other duties as assigned related to the organization’s mission and programs. 

Requirements

  • Knowledge and professional experience conducting home inspections, cost estimating, preparing scopes of work or remediating home-based environmental health issues – particularly mold, lead hazard reduction, weatherization, and/or safety hazards.
  • Minimum of one year of experience in performing home energy audits or in a related field such as a housing inspector required.
  • Bilingual in Spanish and English.
  • High proficiency in Microsoft Office Suite.
  • Excellent organizational and time management skills.
  • Excellent interpersonal communication skills and ability to work both independently and in a team environment.
  • Experience coordinating contractors and subcontractors in conducting housing interventions
  • Professional, positive work ethic and desire to make a difference for families living in Providence, RI.
  • This position will be assigned duties that require the operation of a motor vehicle.  The incumbent must maintain ready access to a motor vehicle, possess a valid motor vehicle operator’s license, and maintain required auto insurance.

Preferred

  • Bachelor’s degree.
  • BPI (Building Performance Institute) Energy Auditor or Building Analyst certification.
  • Rhode Island Pesticide Commercial Applicator Control Applicator License, previous integrated pest management experience, degree in IPM (Integrated Pest Management) related field preferred, or willingness to obtain certification and licensure if needed. 
  • EPA RRP (Renovation, Repair and Painting) and Lead Risk Assessor certified.

The Care Transformation Collaborative of RI (CTC-RI) is excited to announce the upcoming contract for a Lactation Community Program and Policy Specialist, starting February 15 through September 29, with the potential for renewal based on available funding.

This role is a key part of implementing the RI Department of Health’s 2025-2030 Breastfeeding Strategic Plan, developed through the collaborative efforts of HEAL, the WIC, and MCH programs. The plan outlines five priorities: 1) Lactation Workforce, 2) Education and Awareness, 3) Equitable Access to Services, 4) Breastfeeding-Friendly Policies, and 5) Data, Regulations, and Collaboration. CTC-RI will work with healthcare and community partners to address health disparities and improve breastfeeding care across the state, aligning with the CDC’s 2023-2028 State Physical Activity and Nutrition Program cooperative agreement. This exciting initiative marks a significant step in creating lasting change and ensuring better care for breastfeeding families in RI.

Hi everyone,

Many of you know that this newsletter began with my work in Rhode to Equity and initially served as a weekly resource newsletter for the Rhode to Equity teams. It now covers various topics, primarily aimed at helping you build your toolkit and stay informed while sharing opportunities for you to make your voices heard.

In the coming year, I will begin transitioning out of this role. I want to emphasize the importance of continuing our connection and communication, as that is where real change begins. We all need to take action, but it’s not solely our responsibility! Together, we can achieve meaningful change. 😊

  • Our wonderful partner, Mitchelle Abuna at RIDOH, many of you may know from previous focus groups and coding opportunities, is conducting a Community Resources Mapping Survey. We encourage you to take 5-10 minutes to complete this survey. Your participation will help Mitchelle gather data to better understand the distribution, qualifications, and focus areas of CHWs across the state. Thank you for your support! click here.
  • The Olmstead Act is coming to RI and they need our input from those living with disabilities, those who work with people with disabilities, caregivers, family, friends, etc. The two upcoming listening sessions for Olmstead will be held 1/3 and 1/7. See the attached PDFs to share with the community.
    • $50 stipend available!
    • These sessions will serve as a chance to speak about what is working and what is not working in this state for people living with disabilities.

    • Brown University is also holding an in-person session 1/7 and a virtual session on 1/8 5-6:30pm
  • The RI Public Health Association and RIDOH are collaborating on a new data-informed initiative focused on immunizations. This project aims to create community reports based on data to guide and support health promotion efforts, public health messaging, and related initiatives.
    • They are seeking 10-12 participants and aim to start with focus groups in early January via 2-hour Zoom sessions.
    • Compensation of $50 per hour for each participant
    • Currently seeking participants for 3 focus groups in Providence, Block Island, and in the Tribal Community.
    • What to expect: Discussions on experiences related to accessing and receiving vaccines, barriers faced, and ideas for developing and implementing health promotion materials for immunization

  • RIDOH Family Visiting is recruiting for their 2025 Family Visiting Parent/Caregiver Advisory Council – see attached PDF for flyer in English and Spanish
    • Families receive $40 gift cards for attending each monthly session and they ask for 1-2 year commitment.
    • Meetings are virtual 5:30 – 7pm via Zoom and are a mix of providing input on services impacting pregnant people and families with young children.
