Job Summary

The Community Health Worker is a member of the community health team; an interdisciplinary team comprised of Community Health Workers, In-Home team nurse care managers, schedlers, resource specialists, health navigators and nurse practitioners. Works closely with the core team members to support patients who are dealing with complex medical, behavioral health and/or substance issues as well as social determinants of health, and require a more intensive home and community-based intervention.

Duties and Responsibilities

Used to assist the Integra In-Home patients in securing community resources identified through the SDOH screening and in-home assessment.

Acting as a liaison between the patient and the community resource.

Would be responsible for the SDOH screening and assessment of the patients needs to remain in the community.

Assist with the management of the newly implemented chronic condition disease management programs for BCBS members.

The Community Health Worker plays a critical role in assessing patient needs, offering community resources and referrals, providing navigation, support, care coordination, and ongoing case management to meet those needs.

Visits patients in their homes and in the communities in which they live, providing culturally sensitive approaches to health information to improve health literacy.

The Community Health Worker facilitates the patient’s decision-making and self-management to help patients engage in their overall health and achieve their health goals.

This individual will be responsible for tracking patient related activities, monitoring and documenting progress.

Works collaboratively with the primary care team to promote patient-centered care and actively participates in multidisciplinary patient-centered team meetings.

The Community Health Worker has frequent contact with many community agencies on behalf of the patients served, networking and collaborating on resource identification to improve the overall health of the population.


Requirements

High School diploma required with a combination of training and skills to effectively carry out responsibilities and assignments (such as previous experience working with patients in a community-based setting).

Must have Certification in Community Health Work or is working toward certification, must obtain certification within 12 months.

Associates or Bachelor’s degree in Social Work, Community/Public Health or related health sciences field a plus.

Experience working with primary care providers or in other healthcare settings.

Experience working with patients regarding managing their health, navigating systems, providing care coordination and health coaching is preferred.

The ability to travel to various locations in the state and reliable transportation is required.

Must possess a valid, current state issued driver’s license, have reliable transportation and proof of current auto insurance required.

Ability to speak a second language, Spanish preferred.

RICB Updates 4/2/26

On April 7, 2026, Treasure Gallagher, Executive Director of the RICB, led a zoom to share credentialling info and resources with CHWs. Watch a portion of the zoom here.

Key points include:

  • When possible, communicate via email or submit applications online. Try to avoid using the US Postal Service as their current service is 2-3 weeks delayed.
  • Use your personal email when creating an account on the site. Work emails change much more often than personal emails. If you lose access to your email, the RICB cannot get in touch with you regarding updates or expirations. Change you account to your personal email now!
  • Utilize website tools! Once you create an account, within your portal you can download your certification certificate AND upload CEUs. This way as you walk out of a training, you can take a picture of your certificate, upload it, and then it’s done. The date and hours will be recorded for your re-certification records! You can check a certification expiration date under “Certification, Verify a Credential.”
  • “Lapsed Status” ending. Currently (4/2026) a CHW certification lasts two years. There has then been a ‘lapsed’ period of one year before a certification expires. The ‘lapsed’ period is going away at some point in 2026. The RICB will inform us before that happens.
  • Re-Certifying On Time Saves you Money! CHW certifications last two years and must be renewed to stay active. If you renew your CCHW before the two year expiration, the re-certification fee is $75. You may renew as soon as 3 months before your expiration date. After your expiration date, you will be given a 3 month grace period where you may still re-certify. If you renew during that grace period, you will pay the recertification fee of $75 plus a late penalty of $75. (Total of $150). If you fail to renew before the grace period ends, your certification status will become expired. If you CCHW is expired, to re-certify you will need to contact the RICB to receive a re-application packet (similar to your original portfolio application), submit that full application, and pay the re-application fee, currently $125 (4/2026). If your status is expired and you would like to re-apply, email info@ricertboard.org with the subject line: Requesting CCHW Re-Application.
  • Refer to the Domains Regarding Training Eligibility. The RICB fields thousands of emails a day and is not able to respond as quickly as they would like to questions like, ‘Would this training count towards re-certification?’ They encourage you to refer to the CHW Domains (listed on the CCHW application and here) and ask yourself if the training covers material in one or more of those categories. If it does, then it will most likely ‘count.’

Let’s get CHW Funding in the next Federal Budget!

Senator Gillibrand is leading a Dear Colleague Letter in the United States Senate (attached) urging support for CHW funding in the FY 2027 federal appropriations process. Congress has passed bipartisan legislation to authorize $50 million for a CDC grant program for CHWs to promote positive healthy behaviors and outcomes for populations in rural and medically underserved communities. However, this program has not yet been funded. Congress should build on this bipartisan support for CHWs by appropriating  $50 million in the FY 2027 LHHS bill.

What is “appropriations”?

Every year, Congress considers several appropriations bills, which provide funding for federal programs and priorities. An “appropriation” allows a government agency to spend a specified amount of money on a program or service. We can use our power and our voices to ask Congress to include CHWs in the budget! Certain members of Congress sit on the Appropriations Committees in the House and Senate and have more influence in how these funding bills are put together, but other members are also able to push for their funding priorities.

What is a Dear Colleague Letter?

A “Dear Colleague Letter” is a letter sent by members of Congress to other members of Congress. In the appropriations context, a Dear Colleague Letter is most commonly a letter from a group of members of Congress to the Appropriations Committee, asking the Committee to support a particular program. Therefore, members of Congress on the appropriations committees do not usually join appropriations dear colleague letters (since this would be like joining a letter to yourself). As advocates, these letters are a chance to ask as many members of Congress as possible to go on record supporting a program. 

What is this CDC grant program for CHWs mentioned in this Dear Colleague Letter?

In 2010, the Affordable Care Act recognized CHWs and created a CDC grant program to fund CHWs to “promote health in underserved communities.” However, Congress never appropriated the funds for this grant program to offer any awards, meaning this pot was never filled with money. At the end of 2022, Congress passed a bipartisan bill that reauthorized (or, updated) this grant program at a recommended funding level of $50 million. However, Congress still needs to appropriate funds for the CDC to have authority to make awards to CHW programs. While other CDC funding, including grants for specific diseases, do fund CHW work, this grant program is explicitly for CHWs. This means that while the reauthorized grant program passed with a recommended funding level, $0 have been appropriated. In other words, there is now an empty cup for CHWs written into the federal budget with an opportunity to “fill” it to $50 million, but it has yet to be filled. Position Summary

The GED Teacher for the Project LEARN Program provides high-quality, standards-based instruction to adult learners working toward their General Education Diploma (GED). Project LEARN offers free adult education services in a supportive and inclusive learning environment, helping students achieve their educational, career, and workplace goals.

This role delivers multi-level instruction aligned with Rhode Island Department of Education (RIDE) Adult Education Standards and College and Career Readiness Standards (CCRS), using best practices in adult education and learner-centered teaching. The GED Teacher collaborates with program staff to support student assessment, placement, goal setting, academic progress, and documentation. Instruction prepares students for the four official GED subject tests—Math, Language Arts, Science, and Social Studies—administered at approved testing sites, while building the skills and confidence needed for postsecondary success and workforce readiness.

Responsibilities

1) Student Intake, Registration & Assessment

  • Participate in the student registration and intake process in collaboration with program staff.       
  • Administer appropriate CASAS assessments to determine academic functioning levels.       
  • Review assessment results and intake data to evaluate student needs and readiness.       
  • Consult with the instructional and program team to recommend appropriate class placement.       
  • Review intake materials for returning students to ensure continuity of services.

2) Instruction & Program Implementation

  •  Provide multi-level GED instruction aligned with RIDE Adult Education.       
  • Design and implement contextualized, learner-centered lessons that support academic and workplace readiness.       
  • Incorporate a variety of instructional strategies, including whole-group instruction, small groups, paired learning, one-on-one support, dialogue, and role-playing.       
  • Foster a supportive, inclusive classroom environment that respects adult learners’ lived experiences and promotes engagement and persistence.

3) Assessment, Goal Setting & Student Progress       

  • Administer required pre-tests and post-tests in compliance with program and RIDE guidelines.       
  • Develop student learner goals during intake and review progress on a quarterly basis.       
  • Complete student progress reports for alternative learning plans.       
  • Maintain individual student portfolios containing academic work samples and evidence of learning.       
  • Maintain organized student folders with formative assessments, learner goals, testing documentation, and related records.

4) Data, Attendance & Team Collaboration    

  • Complete daily attendance records and submit attendance data to the Data Specialist biweekly.       
  • Communicate timely updates regarding new intakes, attendance, testing, employment outcomes, and documentation.       
  • Share student job placement or employment-related information with the Data Specialist as required.       
  • Maintain consistent communication with program staff to support coordinated service delivery and accurate reporting.

Provide peer support to our homeless, housing-unstable, substance use, and recovery clients in our Safe Haven drop-in center. Maintain a clean, safe, organized, and welcoming space in the drop-in center. Evaluate and assist with clients’ needs and barriers. Ensure program adherence to all relevant licensing, funding, and accreditation standards. Participate in ongoing training and participation in any agency-wide efforts. Preference to bilingual/multi-lingual individuals. Other responsibilities are as follows:

  • Greet clients with unconditional positive regard and develop a trusting working alliance.
  • Assist clients in accessing a wide array of resources, including but not limited to:
  • Food, clothing, personal hygiene products, and survival items.
  • Behavioral health resources.
  • Recovery resources.
  • Emergency shelter resources.
  • Benefits and entitlements.
  • Applications for SSI/SSDI/TDI, etc.
  • Housing resources, including applications for apartments, subsidized housing, universal wait list, etc.
  • Education/employment resources.
  • Family supports.
  • Medical providers; and
  • Resolution of legal barriers.
  • Care coordination with other providers.
  • Connect with external community resources.
  • De-escalate clients experiencing emotional distress.

EDUCATION AND TRAINING REQUIRED:

High school diploma or GED preferred.  Peer Support Certification required.

WORKING CONDITIONS:

Services are provided via the Safe Haven drop-in center. Full-time position, Monday-Friday 8:30-5 pm with some flexibility. Valid driver’s license and reliable transportation required. Must be flexible and able to communicate effectively verbally and in writing.

Peer Recovery Specialist Certification Required

Community Care Alliance is seeking a part time certified peer specialist to work as a member of our 24/7 mobile crisis team. The certified peer specialist will work with a team consisting of a clinician, RN, case manager and community mental health worker and assist individuals who are experiencing a behavioral health crisis. This team responds to crises in the community and assists individuals with immediate access to an assessment and services. The role of the Peer Specialist is to interact with individuals seen and provide a bridge between providers and adults experiencing mental health and/or substance use issues.

The certified peer specialist should be an individual who has lived experience with substance abuse, a behavioral disorder or a mental health disorder who has made progress in their own recovery and has completed specialized training qualifying them to help others in their recovery. This individual should be comfortable honoring multiple pathways to recovery. The Peer Specialist will work as part of a team when providing crisis level of care. The ideal candidate will have the ability to work and communicate effectively and serve as a role model for peers, providing them with coping skills and connecting them to resources in the community. The ideal candidate will be readily able to engage in active supervision and be practice positive and healthy self-care.

Flexible schedule, days, may include evenings and some weekend coverage. 

Education/Skills: 

  • High School diploma or equivalent.
  • Have successfully completed and passed a co- occurring Peer Specialist Training program endorsed by BHDDH and possess or be eligible for the IC/RC state certification.
  • Additionally, the candidate must have a recent work or volunteer history work experience for at least 12 months within the past 3 years.
  • An ideal candidate is someone who can honor a trauma-informed, client-centered care perspective and be accepting of diversity of perspectives and backgrounds. 
  • A valid driver’s license and reliable transportation is required. Bilingual applicants are encouraged to apply.
