Position Summary: This role works with youth and young adults who are in the care and custody of the RI Department of Children, Youth, and Families (DCYF) and living in a residential setting. The Family Support Partner is responsible for working with these youth and young adults and their identified families and kin to promote meaningful and regular contact. The Family Support Partner utilizes lived experience to develop authentic relationships to foster successful collaboration with families and assist to reducing barriers to successful reunification. The Family Support Partner will coordinate family engagement activities to provide a safe, therapeutic environment. May provide direct support and services for youth. The Family Support Partner’s involvement with residents and families is by family choice, as some families may either choose not to have a Family Support Partner or may not require it. The length of involvement is different with each family.

Qualifications:

  • The Family Support Partner must have lived experience of involvement with the RI DCYF.
  • Knowledge and competencies needed to effectively support other families are needed.
  • Trauma Systems Training (TST) training and certification are required, but will be provided in-service at a later date. Must be willing and able to work a flexible schedule, as needed.
  • The position requires reliable transportation, a valid driver’s license, proof of automobile insurance, registration, inspection, and a driving record.
  • In addition, the reliable transportation must have a minimum of three seats in addition to the driver’s seat.
  • Bilingual/ASL skills are compensated by an additional 6%, above base pay.
  • Multilingual skills are compensated by an additional 8%, above base pay.

Position Summary: This position supports survivors of domestic/interpersonal violence and sexual assault (D-IPV/SA) and their families on their journey toward housing security, healing, and self-sufficiency.  Provides housing information, referrals, advocacy, and support in obtaining and maintaining safe and stable housing. Provides coaching, education and triage in tandem with Survivor Support Program Victim Advocate.

Qualifications:

  • High School Diploma with 1-year field experience required; Associate’s Degree or higher in human/social service field preferred.
  • Community Health Worker Certification highly desirable.
  • Experience providing housing support services within Rhode Island preferred.
  • Possession of valid drivers’ license, reliable transportation and proof of current automobile insurance   required.
  • Bilingual English/Spanish required.  FSRI values staff with bilingual language capacity and familiarity with the local community they will be serving.

Job Summary: Bilingual Community Health Worker(CHW) provides peer-to-peer support to families and children with special health care needs; completes family assessments; works with family to determine family goals and implement plan for achieving goals; links families to an array of supports and resources; provides input regarding services and programs in RI and works to mitigate system barriers to accessing services. This CHW works in collaboration with the Newport Public Schools, increasing family/school partnerships, and creating awareness of parental/caregiver rights in the educational success for at-risk and students with disabilities.

Essential Functions:

  • Provide Care Coordination of services and supports, while educating parents and primary caregivers in the navigation of systems of care for their CSHCN, by promoting person-centered care and medical home model
  • Outreach to referred families within agency guidelines
  • Conduct a Family Needs Assessment for referred families and develop an individualized Family Care Plan as appropriate
  • Identify family strengths, concerns and natural supports; utilize motivational interviewing skills and culturally sensitive methods to support members in achieving their goals
  • Support each family in finding geographically convenient supports and services; Collaborate with other service providers, helping families make necessary connections
  • Mentor families encountering special education concerns, assisting families with navigating the education/special education system
  • Assist families as they transition to independence /case closure; provide follow up support as needed
  • Assist in statewide system analysis, planning and coordination with state agencies, state and local boards, community-based organizations, and community rehabilitation programs
  • Actively participate as effective team member in assigned agency meetings and team meetings. Make constructive use of individual and peer supervision/one-on- one meetings
  • Accept other duties and responsibilities as assigned

Qualifications

  • Excellent written and oral communication skills

Education and Experience:

 OVERVIEW

The Treatment Coordinator is responsible for working as a member of the clinical team creating efficient and comprehensive data collection tools to support each child’s treatment program. The data collection tools must be specific and operational to collect all pertinent information that is required to support the child within the intensive home-based program. The Treatment Coordinator must be able to collect, analyze, and interpret all data per child and write progress reports per the standards of care. Swanette Salazar honored with “Hope in Healthcare” Award!

Six RI CHWs including those from the 02907 HEZ and Sankofa Initiatives teams at the West Elmwood Housing Development Corporation attended last weeks Unity Conference in Columbus Ohio. Read NACHW’s recap here.

RI’s new ambassador to NACHW, Lina Roman, spoke about the conference, “This is my second Unity conference but my first time as ambassador.  I met the other ambassadors, one from Alaska, Nebraska, Puerto Rico, New York and New Hampshire. It is interesting and important to see how other states are doing with CHW – do they have training, do they have a state organization? (Nebraska does not.) It helps us understand where Rhode Island is at and where we need to continue to advocate.” 

Outgoing NACHW Ambassador, Swanette Salazar was awarded the ‘Hope In Healthcare Award’ honoring CHWs demonstrating exemplary work towards achieving health equity. She shared, “Unity was such a meaningful experience, and it’s always a joy to be surrounded by my CHW family. This year felt special — I got to share the space with RI’s new ambassador Lina Roman, Geraldine McPhee, and the amazing West Elmwood HEZ leaders, Jessica Thigpen and her team once again!

🎓 CHWARI hits 450!
CHWARI’s Core Training program through RIC’s Institute for Education in Healthcare graduated 31 more CHWs on July 1st! This brings us to more than 450 CHWs trained since 2019. What a strong foundation for the workforce! Stay tuned for dates for our next core training which will be offered in the fall. Priority will be given to those who have already completed some CHW work/volunteer hours, so accrue if you can. 🎓 RI Support Network Celebrates 37 CHW Graduates!
RI Support Network recently graduated 37 CHWs from its 70-hour CHW training program, delivered in partnership with Roger Williams University Extension School. These graduates are now prepared to support individuals and families across Rhode Island through culturally responsive, hands-on health and wellness services.

Thanks to their partnership with RWU Extension School, graduates are eligible to earn 5 college credits toward a degree. 🎓 Visión y Compromiso Promotora Training Graduates 22!
The Latinx CHW Support group collaborated with CHW Voices, and CHWARI to bring the Visión y Compromiso Promotora training to RI. Central Falls School District, Connexion Latina Newport, Bridges Connection Consulting, and Connecting for Children and Families joined the effort to engage Spanish speaking communities across the state. Twenty two promotors participated in 48+ hours of intense, interactive, culturally-appropriate CHW training in Spanish. 

Trainers were given a citation by Representative David Morales, and participants each received citations from Maria Rivera, the Mayor of Central Falls!

Fishing Partnership Health Insurance Navigator

Fishing Partnership Support Service, is a non-profit serving the commercial fishing industry in Massachusetts. We are hiring a CHW for a new office we are opening in Point Judith, RI. This CHW will be focused on improving the health, safety and economic security of commercial fishermen and their families with a primary focus on access to health insurance through the state of RI.  Most of the job is on-the-job training, this person does not need to be a certified CHW, but willing to go through training to get a certification as well as go through the CAC training through RI.  The most important quality this CHW will have that is key to hiring is their connection to the commercial fishing industry in Rhode Island.  We understand this is a specific ask, but it is our model to hire within the industry to provide culture competency in the most holistic way.

Job Description

The RI Health Insurance Navigator provides access to health insurance enrollment for fishermen, fishing families, and fishing communities. This position works within the Community Health department at Fishing Partnership, alongside a team of Community Health Workers (known as Navigators). Our team of Navigators provide commercial fishermen and their families with access to health, safety and economic security programs and services.
ESSENTIAL FUNCTIONS:

  • Support fishermen, their families, and coastal community members with access to health insurance enrollment through Health Source RI, and other state-based programs.
  • Provide fishing families and community members with access to post-enrollment services, including access to providers, support with completing post-enrollment documentation, and more as required.
  • Log meetings, phone calls and interactions in Fishing Partnership database on a weekly basis.
  • Provide outreach for all Fishing Partnership health insurance enrollment programs.
  • Provide referrals to community-based organizations, government supports or other programs and resources (e.g. SHINE, SNAP benefits, or financial resources).
  • Provide outreach for other Fishing Partnership programs in RI, including Safety Training courses.
  • Join fishing industry meetings and Community Health Worker networking opportunities and training as needed.
  • Cultivate relationships with partner organizations and stakeholders through providing information and education regarding Fishing Partnership programs and services, especially access to health insurance.
  • Participate in project teams as needed.
  • Utilize internal Navigator referral process and collaborate with team members to ensure fishermen and consumers served have access to Fishing Partnership programs that are available in RI.

Esta capacitación de 30 horas es válida para el nuevo endoso en Salud Mental y Trastorno por el Uso de Sustancias del RI Certification Board. Un total de 8 Trabajadores Comunitarios de la Salud completaron el curso y ahora están listos para solicitar formalmente el endoso, ampliando sus competencias para apoyar mejor a las comunidades frente a los desafíos relacionados con la salud mental y el uso de sustancias.
