Position Summary: The Family Coaching & Visitation Program Coordinator plays a crucial role in overseeing the efficient operation of the Family Coaching & Visitation (FCV) program. This position involves managing data, coordinating program schedule, ensuring timely reporting, and overseeing the overall workflow of family coaching visitation initiatives. The Program Coordinator will work closely with all FCV staff, maintain datasets, and generate reports to support the successful implementation of the programs.

Family Support Partner, Family Care Community Partnership (FCCP)

Position Summary: Responsible for partnering with families and supporting the wraparound facilitator to do Wraparound with the family.  May provide direct support and services for some families.  Responsible for connecting families with other families with similar challenges and other community resources.  The FSP will partner with and support families with children and youth who are at risk for abuse and neglect, who have serious emotional disturbance (SED) or a developmental disability (DD) and/or who have juvenile corrections involvement in the home and/or school setting.  The FSP’s involvement with families is by family choice, as some families may either choose not to have a FSP or may not require it.  The length of involvement is different with each family as the FSPs role is to empower the family toward self-efficacy. 


Qualifications:

  • The FSP is a peer mentor and must have experience parenting a child with serious emotional disturbance (SED) or a developmental disability (DD) and/or who has been involved with child welfare services or juvenile corrections. 
  • Knowledge and competencies needed to effectively support another parent or caregiver are needed.
  • Wraparound training and certification are required, but will be provided in-service at a later date. 
  • Must be willing and able to work a flexible schedule including evenings and weekends as needed
  • Bilingual/ASL skills are compensated by an additional 6%, above base pay.

Resource Family Navigator, Kinship Cares

Position Summary: Responsible for providing resource services to a client or family in Kinship families. Intervenes in such a manner as to assist the family/client to access necessary services to improve the quality of life. Advocates for clients and family in navigating human service and similar departments, agencies, and services. Connects families to needed services including basic needs, mental health and clinical support, housing assistance, food assistance, etc. Coordinates care for families across multiple service systems. Lead Together Facing the Challenge group for families enrolled in the Kinship CARES services. Provide education and support to parents to develop trauma informed parenting skills. Completes administrative tasks as assigned.


Qualifications: 

  • Bachelor’s degree in human services (psychology, social work, manage/family, etc.) or equivalent combination of education & experience required.

Position Summary: Responsible for providing case management services to young adults living in the program. APRI Wellness Specialist

Position Summary: The AIDS Project RI Wellness Specialist provides holistic, and practical support for people living with HIV. Based out of APRI’s Wellness Center at our Hope Street location, the Wellness Specialist builds relationships with APRI clients in order to keep them connected to care. Central to this role is the administration of our Food Pantry, and Nutritional Supplement Program in collaboration with our Dietician. This is the primary point of encounter for our clients, and a hub of multiple resources for people living with HIV.

The Family Support Partner is responsible for working with these youth and young adults and their identified families and kin to promote meaningful and regular contact. The Family Support Partner utilizes lived experience to develop authentic relationships to foster successful collaboration with families and assist to reducing barriers to successful reunification. The Family Support Partner will coordinate family engagement activities to provide a safe, therapeutic environment. May provide direct support and services for youth. Provides coaching, education and triage in tandem with Survivor Support Program Victim Advocate.

Qualifications:

  • High School Diploma with 1-year field experience required; Associate’s Degree or higher in human/social service field preferred.
  • Community Health Worker Certification highly desirable.

Job Summary: Bilingual Community Health Worker(CHW) provides peer-to-peer support to families and children with special health care needs; completes family assessments; works with family to determine family goals and implement plan for achieving goals; links families to an array of supports and resources; provides input regarding services and programs in RI and works to mitigate system barriers to accessing services. This CHW works in collaboration with the Newport Public Schools, increasing family/school partnerships, and creating awareness of parental/caregiver rights in the educational success for at-risk and students with disabilities.

Essential Functions:

  • Provide Care Coordination of services and supports, while educating parents and primary caregivers in the navigation of systems of care for their CSHCN, by promoting person-centered care and medical home model
  • Outreach to referred families within agency guidelines
  • Conduct a Family Needs Assessment for referred families and develop an individualized Family Care Plan as appropriate
  • Identify family strengths, concerns and natural supports; utilize motivational interviewing skills and culturally sensitive methods to support members in achieving their goals
  • Support each family in finding geographically convenient supports and services; Collaborate with other service providers, helping families make necessary connections
  • Mentor families encountering special education concerns, assisting families with navigating the education/special education system
  • Assist families as they transition to independence /case closure; provide follow up support as needed
  • Assist in statewide system analysis, planning and coordination with state agencies, state and local boards, community-based organizations, and community rehabilitation programs
  • Actively participate as effective team member in assigned agency meetings and team meetings. Make constructive use of individual and peer supervision/one-on- one meetings
