Job Summary: The Bilingual Community Outreach Worker is a non-exempt position located at the Rhode Island Department of Health’s Center for Emergency Preparedness and Response (CEPR). This is a part time position scheduled for 20 hours per week. The Community Outreach Worker will work alongside staff from CEPR to promote the Rhode Island Special Needs Emergency Registry (RISNER) and conduct community outreach activities targeted to assist vulnerable populations in preparing for disasters. At this time, program activities are being held both in-person and virtually.

Essential Functions:

Conduct targeted outreach to explain and promote the Rhode Island Special Needs Emergency Registry (RISNER)

Facilitate RISNER presentations and personal preparedness training to community partners and the general public

Coordinate outreach materials and attend virtual and in-person outreach events (times vary and may include day, evening, and weekend timeframes)

Understand current RISNER utilization and support program strategies for improvement

Independently identify outreach opportunities to increase RISNER enrollment

Attend meetings and trainings as assigned and/or requested by CEPR and the Rhode Island Parent Information Network

Conduct data entry, data cleaning, and basic analysis

Accepts other duties and responsibilities as assigned

Qualifications

Knowledge, Skills, and Abilities:

Knowledge of emergency preparedness, public health and/or populations with access and functional needs

Excellent interpersonal and communications skills, both written and verbal

Experience speaking/presenting to large groups of people and the ability to facilitate group conversation

Ability to work both independently and in collaboration with a team

Proficiency in Microsoft Office Suite (Excel, PowerPoint, Outlook, Teams, etc.) is required

Proficiency in computer and internet navigation (program management tools, online documents, search engine, government websites, etc.)

Fluency (speaking, reading, and writing) in English (required) and Spanish/Portuguese preferred

Education and Experience:

Bachelor’s degree or combination of education and equivalent work experience to meet the needs of the position

Community Health Worker certification preferred; non-certified incumbents are expected to earn certification within 18 months of hire date

Experience that may have been gained through employment in the healthcare or social services fields

Past employment or volunteer experience working in the community

Or, any combination of education and experience that shall be substantially equivalent to the above education and experience

Physical Demands:

While performing the duties of this job, the employee is regularly required to talk or hear. The employee regularly is required to stand, walk, sit, use hand to finger, handle or feel objects, tools, or controls; and reach with hands and arms. Job Summary:

The Bilingual Community Health Worker (CHW) is a peer who has experience in navigating Rhode Island’s health system for themselves, a family member or through previous employment. This CHW will work in RIPIN’s Care Management Program, which supports low income older adults or disabled Rhode Islanders who may be at high risk for nursing facility admission, hospital or institutional care, or homelessness. The CHW will be a critical part of a comprehensive team providing peer support, care coordination, resources and referrals. CHWs will engage with consumers in the home and community settings providing person centered, culturally sensitive support, and building on the values, strengths and preferences of the member. The CHW will also serve as an effective role model and mentor.

Essential Functions:

  • Outreach to consumers in a timely manner in accordance to program guidelines
  • In collaboration with the care team and consumer, assist with the development of a Care Plan to include: actions to improve the consumers’ health status, medical home barriers to care, as well as any social needs identified by the consumer
  • Utilizing motivational interviewing skills and culturally sensitive methods to help consumers achieve the goals of their Care Plan
  • Collaborate with partners to connect consumers with supports and services that will help them live safe, healthy, and independent lives in the community
  • Provide emotional support, serve as a role model, and guide consumers to practice positive, responsible healthy behaviors while remaining fully engaged with each consumer until goals are completed
  • Assist consumers as they transition to independence/case closure by engaging with consumers and providing follow up support
  • Maintain timely, accurate records, documentation, and reports as required
  • Assist in data collection, surveys, assessment and reporting as required
  • Actively participate and complete training and professional development activities
  • Assist in statewide system analysis, planning and coordination with state agencies, state and local boards, community-based organizations, and community rehabilitation programs

Job Summary:

Community Health Worker (CHW) provides peer-to-peer support to families and children with special health care needs; completes family assessments; works with family to determine family goals and implement plan for achieving goals; links families to an array of supports and resources; provides input regarding services and programs in RI and works to mitigate system barriers to accessing services; encourages families to be an active voice in making changes to systems to better meet the needs of families and children with special health care needs.

Essential Functions:

  • Provide Care Coordination of services and supports, while educating parents and primary caregivers in the navigation of systems of care for their CSHCN, by promoting person-centered care and medical home model.
  • Outreach to referred families within agency guidelines of two business days of receiving referral.
  • With each family, conduct a Family Assessment and develop an individualized Family Care Plan within 45 days of referral or sooner, depending on the needs of the child/family

Job Overview:

As a community outreach consultant, you will support the Rhode Island Public Association program coordinator with grant-related community outreach efforts. This is mainly a remote position, but it will involve travel to community events.

On December 4, 2024, the Rhode Island Department of Health (RIDOH) hosted a special summit at District Hall in Providence to celebrate the progress of the CHW Roadmap project including its four key components: CHW Role, Agency, and Leadership; Payment and Sustainability; Person, Place-based Coordination; and Workforce Development for Supervisors and CHWs.

  At the summit, 78 attendees gathered to hear from the four CHW Roadmap teams, who shared their progress, accomplishments, and challenges.

The summit was opened by Jerome Larkin, MD, Director of Health, RIDOH, and Deborah Garneau, MA, Maternal Child Health Director, RIDOH, both of whom emphasized the work of the implementation teams as well as the vital role of the community health worker in Rhode Island. Sherri Ohly, Envision Co-Director, then led a discussion on Sustaining CHW’s in times of change followed by a session on Civic Engagement co-led by Arthur Handy, Representative District 18, State of Rhode Island & Special Project Manager, FSRI and Cristy Romero, CHW, Jenks Park Pediatrics, FSRI.

The day concluded with a world fair which was a valuable opportunity for feedback and collaboration, allowing CHWs and stakeholders to identify key connection points and strategies to propel the work forward. Closing remarks where provided by Kristine Campagna, Associate Director of Health, Division of Community Health and Equity (RIDOH) and James Day, CPS, Health Program Administrator, RIDOH.

CHW Gets Grant to Continue Latinx Support Group!!!!

Congratulations to CCHW Wiston Delgado and the members of RI’s Latinx CHW Support Group! There aren’t enough emojis to show how excited we are!  

This group was started two years ago with seed money from RIDOH’s CDC grant and was supported by CHWARI and our former Bilingual Program Coordinator, Lina Roman.  It was created it to give Spanish speaking CHWs a much needed place to prevent compassion fatigue and share resources in their native language. This fall, when the original funding source expired, Wiston Delgado, CCHW, applied for the funding necessary to keep the group going… and the Rhode Island Foundation’s Juanita Sánchez Community Fund said yes!  

Wiston is excited to share, “I never tried anything like this before, but as a CHW, the thing we do is make connections. I read about the funding source for Rhode Island Latinos in a newsletter. Then I asked the director of a non-profit I knew if they would sponsor my application. They helped me do it.  When the letter came last week I couldn’t believe it!”  A big shout out the Educational Center for Arts & Science (Teatro ECAS) for agreeing to be a non-profit partner with Wiston to make the group eligible for the grant!

Tonya Glantz, of Rhode Island College’s Institute for Eductation in Healthcare and CHWARI, shared, “IEH and CHWARI applaud the continued success of the Latinx support group.  Their dedication to upholding the voices of this group is inspiring and is creating change within the field of community health work.”

The support group’s next meeting will be their in-person holiday gathering on 12/20 from 3pm-5pm. (We originally had it at the wrong time!).  Space is limited but if you would like to attend, you can email Wiston.

They will resume their regular zoom schedule on January 14th at noon. In addition to their meetings they have a very active WhatsApp chat for members to share resources and receive support daily.
